От глубокого инфильтративного эндометриоза к эндометриоз-ассоциированному раку: современные аспекты дифференциальной диагностики

Аннотация
Глубокий инфильтративный эндометриоз является социально значимой проблемой, значительно ухудшающей качество жизни пациенток вследствие хронического болевого синдрома и высокого риска бесплодия. В последнее время все чаще появляются данные о злокачественной трансформации заболевания с формированием очагов малигнизации преимущественно яичниковой, а также внеяичниковой локализации. Сходство клинических проявлений, морфологических характеристик и молекулярных изменений доброкачественных и злокачественных форм существенно затрудняет дифференциальную диагностику. В статье обобщены современные представления о критериях разграничения глубокого инфильтративного эндометриоза и эндометриоз-ассоциированного рака. Подчеркивается ограниченность доказательной базы и необходимость разработки стандартизированных интегративных подходов для повышения точности диагностики и стратификации онкологического риска в данной категории пациенток.
