АНТИФОСФОЛИПИДНЫЙ СИНДРОМ И РАННИЕ РЕПРОДУКТИВНЫЕ ПОТЕРИ

Аннотация

Антифосфолипидные антитела (АФА) многосторонне влияют на процессы формирования и развития трофобласта с момента установления маточно-плацентарного кровотока, когда начинается активный контакт с плазмой матери, содержащей циркулирующие АФА. Открытым и широко обсуждаемым остается вопрос о влиянии АФА на развитие более ранних репродуктивных потерь, то есть на этапе имплантации и инвазии эмбриона, что клинически может проявляться бесплодием неясной этиологии, многократными неудачами экстракорпорального оплодотворения или доклиническими потерями беременности (биохимическими беременностями). Возможными патогенетическими вариантами может быть нарушение процессов ангиогенеза в эндометрии под действием антифосфолипидных антител, повышение активности NK-клеток, усиление протромботических механизмов и десинхронизация процессов фибринолиза и фибринообразования, а также негативное влияние АФА на развитие эмбриона.
