Abstract

With over 70 years of development, the Department of Obstetrics and Gynecology at Tianjin Medical University General Hospital has evolved into a comprehensive center for clinical care, research, and education, showcasing significant influence and technical strength in the field of obstetrics and gynecology. The department operates more than 200 beds, handles over 260,000 outpatient and emergency visits annually, and performs nearly 10,000 surgeries each year. As the final referral and rescue center for complex and critical cases in Tianjin, the department excels in sub-specialties such as perinatal medicine, gynecologic oncology, obstetric critical care, reproductive medicine, and gynecological endocrinology. It has made significant advances in precision diagnosis and personalized treatments, leading the development of the discipline in Northern China. In recent years, the department has demonstrated outstanding scientific innovation, undertaking numerous national and provincial-level projects, publishing high-impact papers, and contributing to multiple international and national guidelines. The department continues to advance both academic and clinical excellence, significantly contributing to the Healthy Tianjin Initiative and the broader Healthy China Action.

Introduction / Введение

With over 70 years of development, the Department of Obstetrics and Gynecology at Tianjin Medical University General Hospital has evolved into a comprehensive disease diagnosis and treatment center with strong expertise, wide influence, a concentration of excellent talents, and robust technical capabilities. It provides safe, convenient, high-quality, and efficient lifelong management and medical services to women in Tianjin and across the nation.

Obstetrics and Gynecology Department / Отделение акушерства и гинекологии

The department currently has more than 200 beds and a staff of over 240 medical, nursing, technical, and research personnel (Fig. 1). It handles 260,000 outpatient and emergency visits annually, performs nearly 10,000 surgeries, and discharges over 10,000 patients each year.

As the final referral and rescue center for complex and critical obstetric and gynecological cases in Tianjin, the department has received numerous national recognitions. These include being named a National Quality Service Unit for Maternal and Infant Safety (2018), one of the first National Key Specialty Units for Menopause Health Care (2019), one of the first National Pilot Units for Ovarian Cancer Standardized Diagnosis and Treatment Quality Control (2023), and being granted approval by the National Health Commission for Preimplantation Genetic Diagnosis technology trials (2023). In 2021, the department established the first integrated clinic for human papilloma virus vaccination and cervical disease diagnosis and treatment in a tertiary general hospital in China (Fig. 2), leading the way in diagnosis, treatment, and service capabilities for women's reproductive health in Tianjin. In recent years, the department has been honored with several prestigious awards, including the National March 8th Red-Banner Collective and the National Women's Civilized Post.

As a comprehensive diagnosis and treatment center for obstetric and gynecological diseases, the department boasts several specialized sub-disciplines.

Reproductive tract infections / Инфекции половых путей

The department has made significant advancements in the precise diagnosis of reproductive tract infections, integration of traditional Chinese and Western medicine, and optimization of treatment models for complex and refractory cases. It has established industry diagnostic standards, trained tens of thousands of obstetricians and gynecologists, and directly benefited millions of patients annually by reducing complications such as infertility, miscarriage, and preterm birth (Fig. 3).

Gynecologic oncology / Гинекологическая онкология

The department has innovatively developed personalized treatment plans for young patients wishing to preserve fertility and elderly patients with severe comorbidities suffering from endometrial cancer. These plans have been promoted to more than 30 institutions nationwide, enhancing the department's influence in the field of gynecologic oncology and advancing the standardization, individualization, and humanization of screening, diagnosis, and treatment of gynecologic cancers across North China and beyond (Fig. 4).

Obstetric critical care / Акушерская интенсивная терапия

As Tianjin's rescue center for critically ill pregnant women, the department has long been responsible for treating complex and severe cases from Tianjin and the North China region. Annually, the department handles and rescues over a thousand critically ill pregnant women, significantly contributing to reducing maternal mortality rates in Tianjin and surrounding areas (Fig. 5). Over the past decade, the department has deepened its collaboration with the hospital's intensive care unit, enhancing its capabilities in cardiopulmonary support and organ protection. The introduction of advanced techniques such as hemodialysis and artificial liver therapy for pregnant women has elevated the department's treatment standards to new heights. The department also features specialized techniques such as multifetal pregnancy reduction, comprehensive treatment of pernicious placenta previa, multidisciplinary joint diagnosis and treatment of hypertensive disorders of pregnancy, diagnosis and treatment of reproductive tract infections during pregnancy, labor analgesia, and intelligent fetal heart rate monitoring systems (Fig. 6).

Reproductive medicine / Репродуктивная медицина

Focusing on addressing fertility issues in older women, the department continues to conduct clinical studies and develop diagnostic and treatment technologies related to eggs, embryos, and the endometrium. The goal is to further improve pregnancy and live birth rates while reducing miscarriage rates in assisted reproductive technologies (Fig. 7, 8).

Gynecological endocrinology and family planning / Гинекологическая эндокринология и планирование семьи

The department provides treatment for abnormal uterine bleeding combined with severe internal diseases, improves comprehensive evaluation and standardized treatment of endometrial diseases, advances menopause health care, and strengthens pelvic floor health care. It conducts in-depth evaluation, prevention, and treatment of pelvic floor disorders (Fig. 9).

Prenatal diagnosis / Пренатальная диагностика

Leveraging its traditional strengths in genetic counseling, prenatal screening, and integrated clinical and laboratory diagnosis for prenatal care, the department has enhanced the precise application of prenatal screening and diagnostic technologies, improving the efficiency of prenatal diagnosis and reducing the occurrence of birth defects (Fig. 10, 11).

Disciplinary development / Развитие дисциплин

  • In 2004, the department was designated as a "Key Discipline" of Tianjin Medical University.
  • In 2014, it was recognized as a "Clinical Key Specialty" of Tianjin Medical University.
  • In 2017, it was approved as a "Key Discipline" by the Tianjin Municipal Education Commission.
  • In 2018, it was designated a "Clinical Key Discipline" by the Tianjin Municipal Health Commission.
  • In 2019, it was recognized as the "Tianjin Key Laboratory for Women's Reproductive Health and Eugenics" and the "Tianjin Branch of the National Clinical Research Center for Obstetrics and Gynecology".
  • In 2021, the department was awarded the title of "Tianjin Medical Key Discipline (Specialty)" and ranked 4th in the Obstetrics and Gynecology specialty reputation ranking in North China according to the "China Hospital Specialty Reputation Ranking".
  • In 2022, it ranked 38th nationwide in the Obstetrics and Gynecology category in the "China Hospital Science and Technology Evaluation Metrics (STEM) Ranking".
  • In 2023, it was approved as a "National Clinical Key Specialty".

Scientific and technological innovation / Научные и технологические инновации

In the past three years, the department has been granted 57 national and provincial-level projects, received 10 provincial-level awards, led or participated in the development of 49 international and national guidelines/consensus statements, and obtained 12 patents, including 5 invention patents. The department has published 103 papers, of which 47 were SCI-indexed, with 12 papers having an impact factor greater than 5 and the highest single paper impact factor reaching 25.476. These publications have been cited 360 times, with a total citation count of 816 for both Chinese and English articles.

Conclusion / Заключение

The Department of Obstetrics and Gynecology will use its designation as a National Clinical Key Specialty as a new starting point, focusing on its specialized sub-disciplines and unique technologies to continuously seek innovation and breakthroughs. The department aims to further elevate its medical service standards and capabilities, contributing to the high-quality development of the hospital and supporting the Healthy Tianjin Initiative and the broader Healthy China Action.
