Актуализирована потребность в длительном приеме препаратов железа и необходимость комплаентности пациенток. Сформулированы основные требования, предъявляемые к препаратам железа на сегодняшний день, а также акцентированы преимущества комбинаций органических солей железа с фолиевой кислотой в качестве наиболее оптимального и современного подхода к профилактике и лечению железодефицитных состояний при беременности.</p></abstract><trans-abstract xml:lang="en"><p>Information published over the last decade on using iron supplements has been collected and systematized. The need for long-term iron supplementation and patient compliance has been updated and emphasized. The main requirements for current iron supplements today have been formulated, and the advantages of combining organic iron salts with folic acid as the most optimal and contemporary approach to the prevention and treatment of iron deficiency during pregnancy have been emphasized.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>железодефицитная анемия</kwd><kwd>фолиевая кислота</kwd><kwd>фумарат железа</kwd><kwd>ферроптоз</kwd><kwd>оксидативный стресс</kwd><kwd>беременность</kwd><kwd>фолаты</kwd><kwd>железо</kwd></kwd-group><kwd-group xml:lang="en"><kwd>iron deficiency anemia</kwd><kwd>folic acid</kwd><kwd>iron fumarate</kwd><kwd>ferroptosis</kwd><kwd>oxidative stress</kwd><kwd>pregnancy</kwd><kwd>folates</kwd><kwd>iron</kwd></kwd-group></article-meta></front><body><sec><title>Введение / Introduction</title><p>Оптимальный баланс микро- и макроэлементов является неотъемлемой частью женского здоровья и особенно важной составляющей физиологического протекания беременности [<xref ref-type="bibr" rid="cit1">1</xref>]. В настоящий момент Всемирная организация здравоохранения (ВОЗ) ставит целью снизить к концу 2025 г. число детей, рожденных с низкой массой тела [<xref ref-type="bibr" rid="cit2">2</xref>]. Разработанная стратегия достижения цели включает в себя многокомпонентную схему, одним из ключевых составляющих которой является улучшение нутритивного статуса матери в период беременности, что во многом определяет не только здоровье самой пациентки и развитие плода, но также и перинатальные исходы [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Нутритивный статус женщины – это система, которая находится в постоянном динамическом равновесии между расходом микро- и макроэлементов на обеспечение протекания физиологических процессов и их биодоступностью в потребляемой пище [<xref ref-type="bibr" rid="cit3">3</xref>]. Последняя находится под влиянием многих факторов: физиологических, географических, культурных, экологических, генетических и этнических [<xref ref-type="bibr" rid="cit3">3</xref>]. Роль микроэлементов, в том числе железа, в организме человека сложно переоценить. Оно обеспечивает течение важнейших биохимических реакций в качестве кофермента, формирует противовоспалительную защиту, а также принимает участие в синтезе белка и регуляции экспрессии генов [<xref ref-type="bibr" rid="cit4">4</xref>]. Во время беременности уровень железа в сыворотке крови женщины может подвергаться значительным колебаниям, однако не должен длительно находиться за пределами референсных значений, ведь его избыток может приводить к усилению окислительного стресса и развитию гемосидероза, в то время как недостаточность способствует прогрессированию железодефицитной анемии (ЖДА), преэклампсии (ПЭ) и, как следствие, патологиям фетоплацентарного комплекса, рождению детей с низкой массой тела и врожденными аномалиями [<xref ref-type="bibr" rid="cit4">4</xref>]. Согласно мировой статистике, именно недостаток железа является самой часто встречающейся нутритивной недостаточностью в мире, наиболее распространенной среди беременных, грудных детей и детей раннего возраста, что обусловлено значительным преобладанием расхода железа над его поступлением в данные периоды жизни [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>По данным международных исследований, дети, подростки и женщины наиболее уязвимы к анемии. Чаще всего от анемии страдают дети и подростки на фоне активного роста, а также женщины в связи с менструальным циклом и беременностью [6–8]: дети младше 5 лет (41,7 %), беременные (40,1 %), женщины в возрасте 15–49 лет (32,5 %) [<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>Согласно официальным статистическим данным, около 40 % женщин по всему миру сталкиваются с диагнозом анемии во время беременности, и ведущей причиной патологического состояния признан дефицит железа и фолиевой кислоты (витамина В9) [<xref ref-type="bibr" rid="cit9">9</xref>][<xref ref-type="bibr" rid="cit10">10</xref>]. Последняя не только играет важную роль в обеспечении процессов формирования нервной трубки плода, но также является основополагающим элементом роста в эмбриональной и фетальной стадиях, неотъемлемой частью системы материнского кроветворения, что определяет дальнейшее полноценное развитие адаптационных систем новорожденного [<xref ref-type="bibr" rid="cit10">10</xref>][<xref ref-type="bibr" rid="cit11">11</xref>]. Поэтому прием фолиевой кислоты (изолированно или наряду с другими витаминами группы В) рекомендуется на этапах как прегравидарной подготовки, так и на ранних сроках гестации [<xref ref-type="bibr" rid="cit11">11</xref>]. Доказано, что снижение содержания железа и фолиевой кислоты во время беременности ассоциировано не только с анемией, но также с риском геморрагических заболеваний и пороков развития плода [<xref ref-type="bibr" rid="cit12">12</xref>]. В условиях алиментарного недостатка железа у матери, который неминуемо развивается в связи с увеличивающимися вдвое потребностями в данном микроэлементе [<xref ref-type="bibr" rid="cit11">11</xref>], развивается состояние, которое четко коррелирует с неблагоприятными исходами акушерских кровотечений, возрастающей частотой преждевременного разрыва плодных оболочек, быстрым истощением физических сил женщины в родах, а также увеличением материнской смертности [<xref ref-type="bibr" rid="cit13">13</xref>]. Исследования роли эпигенетических изменений в организме формирующегося плода с применением так называемых «омиксных» технологий способствовали открытию вклада недостаточности фолиевой кислоты в программирование хронических заболеваний у ребенка [<xref ref-type="bibr" rid="cit14">14</xref>].</p><p>В идеале дефицит питательных микронутриенов следует купировать еще на этапе прегравидарной подготовки. Именно поэтому разумным подходом является введение в рацион женщин, имеющих ближайшие репродуктивные планы, комплекса биологически активных добавок с последующим продолжением их применения во время наступившей беременности с целью профилактики возникновения дефицита микро- и макроэлементов [<xref ref-type="bibr" rid="cit14">14</xref>]. Данный подход целесообразен, в первую очередь, потому что истощенные запасы железа у матери в период беременности достаточно трудно купировать, а также в связи с тем, что фолиевая кислота имеет критически важное значение на ранних сроках гестации (21–26 дней) [<xref ref-type="bibr" rid="cit14">14</xref>].</p></sec><sec><title>Основные понятия / Basic concepts</title><p>Выделяют несколько стадий железодефицита:</p><p>– предлатентный дефицит железа, характеризующийся снижением запасов железа без изменения ключевых показателей гемопоэза;</p><p>– латентный дефицит железа (ЛДЖ), определяемый как полное истощение депо микроэлемента в организме без проявлений признаков анемизации как клинически, так и в лабораторных показателях красной крови. В настоящее время доказано, что длительно существующий ЛДЖ даже без перехода в стадию анемии достоверно ассоциирован с повышенным риском смертности пациентов любого возраста [<xref ref-type="bibr" rid="cit15">15</xref>];</p><p>– манифестная ЖДА – приобретенное заболевание, характеризующееся снижением содержания железа в сыворотке крови, костном мозге и тканевых депо, клинически проявляющееся характерными синдромами (анемическим, сидеропеническим, гематологическим), с нарушением процессов синтеза гема и образования гемоглобина, и как следствие, развитием гипохромии эритроцитов и трофических расстройств в тканях в результате циркуляторной гипоксии [<xref ref-type="bibr" rid="cit16">16</xref>].</p><p>Рутинно обследовать на дефицит железа имеет смысл пациентов, относящихся к группам риска [<xref ref-type="bibr" rid="cit17">17</xref>]: имеющих ранее установленный диагноз дефицита железа; диабет; ВИЧ инфекцию; воспалительные заболевания кишечника; несколько родов в анамнезе, особенно если интервал от родов до беременности меньше 6 месяцев; аномальные маточные кровотечения в прошлом; обильные менструации; дефицит массы тела или ожирение; курящих; вегетарианцев.</p></sec><sec><title>Ключевые аспекты метаболизма железа в организме / Key aspects related to in vivo iron metabolism</title><p>Основное внимание акушеров-гинекологов сегодня должно быть сконцентрировано на поиске препаратов с оптимальным, научно обоснованным соотношением микронутриентов и их дозировок [<xref ref-type="bibr" rid="cit18">18</xref>]. Обоснование целесообразности назначения того или иного препарата невозможно без понимания метаболизма железа в организме и его особенностей в период гестации. Улучшение понимания обмена веществ и разработка новых препаратов железа, которые лучше усваиваются и одновременно снижают частоту развития побочных эффектов, могут повысить терапевтическую эффективность перорального приема железа ввиду повышения комплаентности [<xref ref-type="bibr" rid="cit19">19</xref>]. За последние 10–15 лет ряд открытий способствовали появлению новых взглядов на процессы формирования железодефицита.</p><p>Необходимо разделять понятия абсолютного и функционального железодефицита. В первом случае нехватка микронутриента обусловлена хронической кровопотерей (применительно к сфере акушерства чаще всего аномальными маточными кровотечениями, возникающими вследствие ряда причин), а также повышенным расходом железа в период беременности и лактации, в то время как функциональный дефицит железа возникает при нормальном насыщении депо, но при нарушении процессов метаболизма по разным причинам [<xref ref-type="bibr" rid="cit20">20</xref>].</p><p>Поскольку в организме человека не существует механизма, регулирующего выведение железа из организма, как избыток, так и недостаток его экзогенного потребления отрицательно влияют на процесс фосфорилирования митохондрий и, следовательно, энергообеспечения клеток [<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit22">22</xref>]. Таким образом, как и для большинства нутриентов, содержание железа в организме формирует U-образную кривую риска [<xref ref-type="bibr" rid="cit23">23</xref>]. Более того, открытие понятия ферроптоза доказывает необходимость особенно тщательного подбора дозировки лекарственных препаратов, содержащих железо. Термин ферроптоз был предложен в 2012 г. Скоттом Диксоном [<xref ref-type="bibr" rid="cit24">24</xref>]. Ферроптоз можно определить, как особую железо-зависимую форму клеточного некроза, обусловленную массивным повреждением клеточных мембран активными формами кислорода (АФК), что приводит к перекисному окислению липидов, оказывает цитотоксический и мутагенный эффект [<xref ref-type="bibr" rid="cit25">25</xref>][<xref ref-type="bibr" rid="cit26">26</xref>]. Ключевую роль в возникновении ферроптоза играет реакция Фентона, в результате которой образуется большое количество свободных радикалов, запускающих перекисное окисление липидов, что в итоге приводит к клеточной гибели в форме ферроптоза и железо-ассоциированной аутофагии [<xref ref-type="bibr" rid="cit25">25</xref>][<xref ref-type="bibr" rid="cit27">27</xref>]. Таким образом, оптимальный баланс железа применительно к сфере пероральных препаратов в первую очередь определяется их дозировкой и кратностью приема, имеет решающее значение для оптимального функционирования клеток. Можно сделать вывод о преимуществе препаратов, содержащих железо в низких дозировках, перед традиционными высокодозированными ввиду возможности обеспечения постоянной концентрации железа в крови без резких пиков, во время которых может возникнуть риск перегрузки железом, ферроптоза и сидероза. Дополнительно отсутствие высоких концентраций железа в препаратах уменьшает раздражающее действие на слизистую желудочно-кишечного тракта (ЖКТ), что ведет к снижению частоты побочных реакций и, следовательно, повышению комплаентности.</p><p>Ключом к подбору оптимальной дозировки и кратности приема препаратов железа является понимание его метаболизма. В данных процессах задействованы 3 основных типа клеток: энтероциты тонкого кишечника, обеспечивающие всасывание железа из просвета двенадцатиперстной кишки с помощью ферропортина [<xref ref-type="bibr" rid="cit28">28</xref>][<xref ref-type="bibr" rid="cit29">29</xref>]; гепатоциты, участвующие в формировании депо железа; макрофаги, или правильнее будет назвать их сидерофаги, поддерживающие циркуляцию железа между печенью, селезенкой и красным костным мозгом [<xref ref-type="bibr" rid="cit26">26</xref>]. Последние играют некую роль «быстрого доступа» запасов железа в органы-мишени (печень, селезенку, красный костный мозг) в случае необходимости [<xref ref-type="bibr" rid="cit28">28</xref>].</p><p>Транспорт железа между органами и тканями осуществляется с помощью плазменных белков-переносчиков, основным из которых является трансферрин. Рецепторы трансферрина экспрессируются в каждой клетке организма, способствуя при связи с соответствующим белком образованию эндосомы и пинацетозу железа в клетку [<xref ref-type="bibr" rid="cit29">29</xref>]. В норме содержание в плазме трансферрина составляет около 30 %, что обеспечивает оптимальную буферную емкость в случае потенциальной перегрузки железом [<xref ref-type="bibr" rid="cit30">30</xref>].</p><p>При пероральном приеме препаратов железа наибольшего внимания заслуживают факторы, способные повлиять на процессы его всасывания в ЖКТ. В основном абсорбция железа из пищи осуществляется ворсинками зрелых энтероцитов в проксимальном отделе тонкого кишечника, где низкий уровень рН способствует его сохранению в растворимой форме, делая доступным для усвоения. Лучшими показателями всасываемости обладают препараты двухвалентного железа в сравнении с трехвалентным. В физиологических условиях окисление Fe3+ до Fe2+ происходит под воздействием редуктаз, таких как цитохром В, прежде чем оно будет импортировано на апикальную мембрану энтероцита [<xref ref-type="bibr" rid="cit31">31</xref>]. Дальнейшее связывание с трансферрином плазмы возможно также исключительно для двухвалентного железа. Таким образом, в вопросах пероральных форм препаратов железа целесообразно использовать уже восстановленные до двухвалентного железа соединения.
</p><p>Дальнейшая судьба железа в клетках организма напрямую зависит от их энергетических потребностей. В случае дефицита ионы железа немедленно транспортируются к митохондриям, включаясь в процессы окислительного фосфорилирования и энергообеспечения [<xref ref-type="bibr" rid="cit32">32</xref>]. Второй путь – запасание микроэлемента в депо в виде ферритина, создавая резервуар для последующего использования [<xref ref-type="bibr" rid="cit32">32</xref>]. Поскольку свободное железо в растворенном виде весьма опасно для клеток, существует система его внутриклеточного транспорта в виде белков-шаперонов, одним из которых является rC-связывающий белок (англ.poly(rC)-binding protein 1, PCBP1), обладающий способностью переносить железо к ферритину, предохраняя клетку от возможной реакции свободнорадикального окисления [<xref ref-type="bibr" rid="cit32">32</xref>].</p><p>Всасывание и распределение железа в тканях принципиально определяется активностью гепсидина – циркулирующего гормона белковой природы, поддерживающего гемостаз путем регулирования деградации ферропортина, являющегося единственным ныне известным клеточным экспортером железа [32–34]. Чаще он продуцируется гепатоцитами в ответ на недостаток железа, однако в условиях воспаления функционирует как один из белков острой фазы, преимущественно воздействуя через интерлейкин (англ. interleukin, IL) IL-6 [<xref ref-type="bibr" rid="cit35">35</xref>]. В настоящее время описаны 2 основных механизма взаимодействия ферропортина и гепсидина: гепсидин-индуцированный эндоцитоз и разрушение ферропортина макрофагами, а также прямое связывание с ферропортином, изменение конфигурации последнего и полное блокирование выхода железа из клетки, преимущественно в энтероцитах [36–38]. Результатом вышеописанных реакций является увеличение процентного содержания внутриклеточного железа и снижение его концентрации в плазме крови [<xref ref-type="bibr" rid="cit32">32</xref>][<xref ref-type="bibr" rid="cit39">39</xref>]. Гепсидин, как и остальные гормоны, действует по принципу обратной связи [<xref ref-type="bibr" rid="cit32">32</xref>]. Его выработка уменьшается при дефиците железа и, напротив, увеличивается в условиях клеточной перегрузки железом и воспаления с целью защиты клеток от токсического повреждения свободными радикалами [<xref ref-type="bibr" rid="cit32">32</xref>][<xref ref-type="bibr" rid="cit40">40</xref>]. Таким образом, повышение уровня гепсидина, способного системно блокировать клеточные транспортеры железа, косвенно приводит к ухудшению всасывания железа из просвета двенадцатиперстной кишки, что требует пересмотреть концепцию применения высоких доз препаратов железа.</p><p>С учетом роли гепсидина и повышенной выработки его в ответ на экзогенное поступление железа, регулируемой по принципу обратной связи, открыт вопрос о необходимости 2-3-кратного приема препаратов в течение дня в более низких дозировках, неспособных создавать условия клеточной перегрузки железом.</p></sec><sec><title>Особенности метаболизма железа в период беременности / Features of iron metabolism during pregnancy</title><p>Расход железа в течение беременности: 300 мг – плод и плацента, 500 мг – увеличение количества эритроцитов, 300 мг – расход кожей и кишечником [<xref ref-type="bibr" rid="cit41">41</xref>].</p><p>Важно вовремя диагностировать и корригировать дефицит железа, в противном случае мать и плод столкнутся с негативными последствиями [<xref ref-type="bibr" rid="cit42">42</xref>]:</p><p>Соли двухвалетного железа являются препаратами выбора для коррекции и профилактики дефицита железа [<xref ref-type="bibr" rid="cit42">42</xref>].</p><p>Ввиду прямой корреляции дефицита железа во время беременности с неблагоприятными перинатальными исходами как для матери, так и для ребенка, его дополнительный прием рекомендован в абсолютном большинстве источников [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>]. В среднем, чтобы покрыть повышенные потребности в данном микроэлементе, женщине рекомендуется прием около 1,0 г дополнительного железа в период беременности ежедневно – это соответствующая диета и дотация с помощью лекарственных препаратов в соответствии с установленными нормами для элементарного железа [<xref ref-type="bibr" rid="cit45">45</xref>]. Недостаточность поступления железа к плоду достоверно ассоциирована с такими патологическими состояниями, как нарушение развития структур головного мозга, что в последующем приводит к депрессии, патологической злости, аутизму, проблемам с памятью и эмоциональной лабильности вплоть до шизофрении в раннем детском возрасте [<xref ref-type="bibr" rid="cit46">46</xref>]. Предположительно, это связано с нарушением процесса миелинизации вещества головного мозга [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. Со стороны матери основными рисками железодефицита являются ПЭ, тяжелые послеродовые кровотечения, материнская смертность, преждевременные роды, задержка внутриутробного развития плода [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>].</p><p>Так как проблема дефицита железа во время беременности актуальна как для матери, так и для ребенка, возникает вопрос: кому из них двоих в большей степени достаются запасы микроэлемента? Всем известен факт, что в описанной ситуации, плод играет роль «паразита» для матери, так как продолжает поглощать железо в необходимых ему количествах, несмотря на истощение «макроорганизма» [<xref ref-type="bibr" rid="cit32">32</xref>]. Основным регулятором поступления железа к плоду является плацента, деятельность которой находится под воздействием материнских, фетальных и аутокринных сигналов [<xref ref-type="bibr" rid="cit32">32</xref>]. В организме человека ключевую роль в транспорте железа от матери к плоду играет синцитиотрофобласт [<xref ref-type="bibr" rid="cit32">32</xref>]. Своего рода «экзогенным» источником железа для плода является не гемовое железо, поступающее с пищей, как у взрослых, а трансферрин-связанное железо материнского кровотока, омывающего ворсинки хориона. Согласно наиболее распространенной теории, железо импортируется в клетки синцитиотрофобласта с помощью пинацетоза, который инициируется путем возбуждения рецепторов к трансферрину 1-го типа (англ. transferrin receptor type 1, TfR1), расположенных на апикальной поверхности ворсинок [<xref ref-type="bibr" rid="cit47">47</xref>]. После попадания в цитозоль клеток хориона железо может либо запасаться в плаценте в качестве депо, либо сразу экспортироваться в кровоток плода через каналы базального слоя синцитиотрофобласта, контактирующего с эндотелием капилляров плода [<xref ref-type="bibr" rid="cit48">48</xref>][<xref ref-type="bibr" rid="cit49">49</xref>]. В настоящий момент тонкие молекулярные механизмы транспорта железа через эндотелий капилляров плода требуют дальнейших исследований.</p><p>Активность транспорта железа к плоду в условиях его дефицита и конкурентной борьбы между матерью и ребенком напрямую регулируется плацентой, в основном путем изменения экспрессии белков-транспортеров железа на поверхности клеток синцитиотрофобласта [<xref ref-type="bibr" rid="cit32">32</xref>]. Было доказано, что в условиях дефицита железа у матери экспрессия плацентарного TfR1 увеличивается [<xref ref-type="bibr" rid="cit50">50</xref>]. Данный процесс является своего рода компенсаторным механизмом, используемым для обеспечения адекватной доставки железа плоду [<xref ref-type="bibr" rid="cit32">32</xref>].</p><p>В условиях возрастающей потребности в железе по мере увеличения срока гестации его необходимый объем может быть усвоен из рациона или мобилизован из депо [<xref ref-type="bibr" rid="cit51">51</xref>]. Данные процессы во время беременности опосредуются гормоном-регулятором гепсидином, уровень которого доказано снижен на поздних сроках [<xref ref-type="bibr" rid="cit52">52</xref>], что приводит к усилению всасывания железа энтероцитами тонкого кишечника, а также усилению экспорта железа из депо ферритина в системный кровоток. Что касается фетального гепсидина, его предположительная роль характеризуется противоположным эффектом, заключающимся в аутокринной/паракринной стимуляции ферропортина гепатоцитов, что приводит, напротив, к сохранению запасов железа в печени плода и косвенной стимуляции эритропоэза [<xref ref-type="bibr" rid="cit32">32</xref>].</p><p>Железо также критически важно для других ферментов и белков, участвующих в выработке энергии и функционировании клеток. Есть данные, что этот распространенный дефицит может привести к неблагоприятному воздействию на щитовидную железу, особенно на функцию фермента тиреопероксидазы [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit53">53</xref>]. Кроме того, анемия, вызванная дефицитом железа, является распространенным сопутствующим заболеванием у пациентов с заболеваниями щитовидной железы, встречается почти у 5 % населения [<xref ref-type="bibr" rid="cit54">54</xref>]. В международном медицинском сообществе активно обсуждается возможная роль дефицита железа в патогенезе заболеваний щитовидной железы. В нескольких исследованиях сообщалось о высокой распространенности дефицита железа у пациентов с заболеваниями щитовидной железы, в частности, гипотиреозом в исходе аутоиммунного тиреоидита [<xref ref-type="bibr" rid="cit55">55</xref>]. Дефицит железа отрицательно влияет на выработку гормонов щитовидной железы, а их дефицит снижает пролиферацию предшественников эритроцитов как напрямую, так и через снижение секреции эритропоэтина почками [<xref ref-type="bibr" rid="cit56">56</xref>][<xref ref-type="bibr" rid="cit57">57</xref>]. Кроме этого, дефицит железа может влиять на ось гипоталамус-гипофиз-щитовидная железа, что приводит к изменению уровня гормонов щитовидной железы и снижению ответа на тиреотропный гормон. Железо также необходимо для активности тиреопероксидазы, фермента, который катализирует йодирование остатков тирозина в тиреоглобулине, белке-предшественнике синтеза гормонов щитовидной железы [<xref ref-type="bibr" rid="cit58">58</xref>]. Исследования на животных показали, что дефицит железа может влиять на активность тироксиндейодиназы, снижая превращение тироксина (Т4) в трийодтиронин (Т3), и на регуляцию метаболизма гормонов щитовидной железы на центральном уровне [<xref ref-type="bibr" rid="cit59">59</xref>][<xref ref-type="bibr" rid="cit60">60</xref>].</p><p>Несмотря на растущий интерес к взаимосвязи дефицита железа и дисфункции щитовидной железы, научные данные пока не позволяют однозначно определить связь между уровнем железа и функцией щитовидной железы. Было проведено несколько исследований для оценки связи между уровнем железа и функцией щитовидной железы, но результаты противоречивы. В то время как некоторые исследования предполагают, что дефицит железа связан с повышенным риском дисфункции щитовидной железы, другие не обнаружили значимой связи. Недавний систематический обзор [<xref ref-type="bibr" rid="cit61">61</xref>] и метаанализ показывают, что распространенность явного и субклинического гипотиреоза выше у беременных и женщин детородного возраста с дефицитом железа, у которых были более высокие значения тиреотропного гормона (ТТГ) и сниженные значения свободного тироксина (англ. free thyroxine, FT4) [<xref ref-type="bibr" rid="cit62">62</xref>].</p></sec><sec><title>Роль фолиевой кислоты во время беременности / The role of folic acid in pregnancy</title><p>Установлено, что во время беременности потребность организма женщины в фолатах возрастает в 3–6 раз [<xref ref-type="bibr" rid="cit63">63</xref>]. Дефицит фолиевой кислоты способен негативно влиять на метаболизм железа в данный период, что может отразиться на здоровье как матери, так и плода. В настоящее время ВОЗ разработала стратегию, согласно которой минимальная обязательная потребность в дополнительном ежедневном приеме железа во время беременности составляет до 60 мг, а фолиевой кислоты – 0,4 мг с целью покрыть возрастающие потребности [<xref ref-type="bibr" rid="cit64">64</xref>][<xref ref-type="bibr" rid="cit65">65</xref>]. Международные рекомендуемые нормы потребления фолиевой кислоты [<xref ref-type="bibr" rid="cit66">66</xref>] Национальных академий наук, инженерии и медицины (англ. National Academies of Sciences, Engineering, and Medicine):</p><p>Доказано, что именно комбинация, а не отдельный пероральный прием указанных препаратов способствует снижению материнской смертности, частоты послеродовых септических осложнений, анемизации беременных, рождения детей с низкой массой тела и преждевременных родов [<xref ref-type="bibr" rid="cit68">68</xref>]. Группы риска женщин с высоким риском могут получить пользу от ежедневных доз фолиевой кислоты выше, чем 0,8–0,9 мг/день, как в большинстве пренатальных витаминов, а именно, от дозы 5 мг [<xref ref-type="bibr" rid="cit69">69</xref>][<xref ref-type="bibr" rid="cit70">70</xref>].</p><p>Группы риска: женщины, у которых в анамнезе рождение детей с дефектами нервной трубки (ДНТ); определенные генотипы, связанные с метаболизмом фолиевой кислоты; женщины с сахарным диабетом (СД); с синдромом мальабсорбции; принимающие антифолатные препараты; с ожирением; курящие; с доказанной или предполагаемой низкой приверженностью к приему добавок фолиевой кислоты [<xref ref-type="bibr" rid="cit70">70</xref>].</p><p>Согласно рекомендациям Международной федерации акушеров и гинекологов (англ. International Federation of Gynecology and Obstetrics, FIGO) [<xref ref-type="bibr" rid="cit69">69</xref>]:</p><p>– 4 мг в сутки при индексе массы тела (ИМТ) &gt; 35, ДНТ в анамнезе;</p><p>– 5 мг в сутки при СД, противосудорожном лечении.</p><p>Фолиевая кислота является синтетической формой витамина В9 и играет критически важную роль в таких процессах, как превращение гомоцистеина в метионин, а также совместно с витамином В12 опосредует синтез нуклеотидов, что является неотъемлемой составляющей оптимального протекания процессов эритропоэза, обмена аминокислот, метилирования транспортной рибонуклеиновой кислоты (англ. transfer ribonucleic acid, tRNA) и др. [<xref ref-type="bibr" rid="cit71">71</xref>]. Пуриновые и пиримидиновые основания способствуют образованию спиральной структуры молекулы ДНК и оптимальному протеканию интенсивных процессов клеточного деления в организме плода, поэтому дефицит фолатов способен не только негативно повлиять на процессы формирования нервной трубки и сердца вплоть до формирования пороков при дефиците фолиевой кислоты на 5–8-й неделе эмбриогенеза, но также и вызывать у матери онкологические заболевания и нарушение иммунной функции [<xref ref-type="bibr" rid="cit63">63</xref>][<xref ref-type="bibr" rid="cit71">71</xref>]. Доказано, что дефицит фолиевой кислоты способен также усугубить окислительный стресс, ассоциированный с железом, следующим образом: аминокислота гомоцистеин содержит в своей структуре тиоловую группу, которая обладает высокой реактогенностью и способна быстро подвергаться окислению в присутствии железа, кислорода, меди с образованием потенциальных АФК [<xref ref-type="bibr" rid="cit71">71</xref>]. Присутствие фолиевой кислоты в рационе способно подавить отложение избытка железа в печени и других органах, профилактируя возможность развития гемосидероза [<xref ref-type="bibr" rid="cit72">72</xref>]. Комбинация перорального приема препаратов железа с фолатами способна снизить потенциальный риск ферроптоза, делая прием более безопасным. Было установлено, что именно совместный прием указанных нутриентов способен оказывать благоприятное действие на организм матери, снижая уровни креатинина, глюкозы и кровяного давления, таким образом снижая риски гестационного СД, острой почечной недостаточности и ПЭ [<xref ref-type="bibr" rid="cit71">71</xref>].</p><p>Железо-фолатная комбинация во время беременности способна снизить действие окислительного стресса, улучшить метаболизм, гемопоэз, благоприятно влиять на функцию почек и сердца, снизить до оптимальных цифр прибавку массы тела матери во время беременности и улучшить основные перинатальные исходы, безопасна при длительном приеме. Фолиевая кислота – водорастворимый витамин, не накапливается, не создает депо в организме, содержится в минимальных количествах. Дефицит фолиевой кислоты может быть вызван недостаточным потреблением, нарушением всасывания или повышенными метаболическими потребностями, как, например, при беременности, гемолитической анемии и в периоды быстрого роста. Поскольку запасы фолиевой кислоты в организме ограничены (примерно 0,5–20 мг), а суточная потребность составляет около 400 мкг, дефицит может развиться в течение 8–16 недель. Начинать прием фолатов нужно заблаговременно до беременности, так как запасы фолиевой кислоты быстро заканчиваются. Беременность, наступившая на фоне дефицита фолиевой кислоты, несет за собой серьезные риски [<xref ref-type="bibr" rid="cit73">73</xref>].</p></sec><sec><title>Повышение приверженности пациенток при длительном лечении пероральными препаратами железа / Increased patient adherence in long-term oral iron therapy</title><p>С позиции доказательной медицины и широкой доступности информации как для врачей, так и для пациенток необходимость дополнительного приема препаратов железа и фолиевой кислоты во время беременности уже не вызывает сомнений. Как отмечает ВОЗ, разработанная стратегия длительной профилактики и лечения железодефицита сталкивается лишь с одной проблемой – низкой комплаентностью пациенток ввиду частого возникновения нежелательных побочных эффектов, что делает невозможным прием препаратов на постоянной основе [<xref ref-type="bibr" rid="cit74">74</xref>]. Наиболее частыми из них являются местные побочные реакции в ЖКТ, обусловленные раздражающим действием на слизистую оболочку, что проявляется нарушением стула, диспепсическими явлениями, неприятным привкусом во рту, а в период беременности усугублением рвотного рефлекса. В связи с этим в последнее время в литературе опубликован ряд исследований, демонстрирующих улучшенную переносимость препаратов железа в виде органических солей в сравнении с неорганическими ввиду их уменьшенной токсичности, что обусловливает возможность их приема даже на фоне токсикоза и гастритов [<xref ref-type="bibr" rid="cit75">75</xref>][<xref ref-type="bibr" rid="cit76">76</xref>]. В условиях улучшенной переносимости становится возможным прием препаратов железа длительным курсом, что позволяет оптимизировать терапию анемии.</p><p>Современное лечение препаратами железа должно быть длительным и состоять из 2 этапов: нормализации уровня гемоглобина и показателей красной крови с последующей коррекцией уровня железа в депо с целью устранения латентного железодефицита. Опубликованы доказательства того, что пероральные препараты, содержащие низкие дозы железа, применяемые короткими курсами по 2 недели ежемесячно или же через день на протяжении нескольких месяцев, характеризуются более высокой эффективностью и снижением количества и степени выраженности побочных эффектов в сравнении с высокодозированными препаратами, применяемыми по 2–3 раза в день [<xref ref-type="bibr" rid="cit77">77</xref>].</p><p>Согласно недавно опубликованному пилотному исследованию, показатели приверженности к лечению были значительно ниже в подгруппах, принимавших препараты железа ежедневно, что подтверждает целесообразность приема в альтернативном режиме. Так, комплаентность пациенток при ежедневном приеме таблеток составила 47 %, приеме через день – 62 %, а при приеме 3 раза в неделю – 61 %. Двумя основными причинами пропуска приема лекарств были развитие побочных эффектов, а также забывчивость. При этом средние значения гемоглобина, измеряемые еженедельно, между группами не имели статистически значимых различий. Было показано, что частота неблагоприятных исходов беременности не коррелирует с режимом приема и дозировкой препарата [<xref ref-type="bibr" rid="cit78">78</xref>].</p></sec><sec><title>Заключение / Conclusion</title><p>Таким образом, основными требованиями, предъявляемыми к препаратам железа, особенно на этапе прегравидарной подготовки, являются:</p><p>– препарат должен обеспечивать доставку железа к непосредственному месту всасывания – двенадцатиперстной кишке в неизмененном виде;</p><p>– оптимален комбинированный прием препаратов железа и фолиевой кислоты, так как фолаты способны улучшать абсорбцию железа, оказывать позитивное действие на гемопоэз, демонстрируют протективный антиоксидантный эффект, снижают риски возникновения ПЭ, острого почечного повреждения и гестационного СД;</p><p>– предпочтительны препараты двухвалентного железа ввиду лучшей абсорбции в ЖКТ;</p><p>– органические соли железа обладают меньшей токсичностью и имеют преимущество перед неорганическими соединениями ввиду меньшей частоты развития побочных эффектов;</p><p>– более низкие дозировки железа позволяют снизить частоту побочных эффектов и повысить комплаентность без уменьшения эффективности терапии;</p><p>– отсутствие неприятного вкуса и запаха, что также может повысить приверженность к лечению;</p><p>– отсутствие контакта действующих веществ с полостью рта, предупреждающее неблагоприятный косметический эффект в виде окрашивания зубной эмали.</p></sec></body><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Gopal M., Sunitha K., Arockiasamy J. et al.
