Также многочисленные исследования показали, что ГГЦ является фактором риска реализации таких серьезных акушерских осложнений, как привычное невынашивание беременности, преждевременные роды, преэклампсия, преждевременная отслойка плаценты, задержка развития плода, гестационный сахарный диабет. В тоже время, несмотря на множество клинических и экспериментальных исследований, значимость и многогранность эффектов ГГЦ продолжает поддерживать интерес исследователей к проблематике. В последние годы получены данные о малоизвестных явлениях, ассоциированных с ГГЦ матери при беременности, таких как «эпигенетическое старение плода» и «плодовый атерогенез». Эпигенетическое старение – это индивидуальный показатель старения, который фиксирует межиндивидуальные различия во времени начала какого-либо заболевания на протяжении всей жизни, оно имеет место во внутриутробном развитии и выражается в гипометилировании ДНК. Умеренная ГГЦ при беременности вызывает гипометилирование ДНК у плода, что, согласно результатам последних исследований, может являться основным механизмом, определяющим здоровье в детском и взрослом возрасте. «Плодовый атерогенез» – это еще один малоизученный эффект ГГЦ. Данный термин подчеркивает, что формирование атеросклеротических бляшек начинается не во взрослом возрасте, а внутриутробно. Существует ряд исследований, которые показали, что образование утолщений и жировых прожилок интимы сосудов, участвующих в патогенезе атеросклероза, начинается антенатально. Открытие новых аспектов негативного влияния ГГЦ при беременности на развитие плода обуславливает необходимость подробного изучения причин ГГЦ как немодифицируемых (генетические дефекты ферментных систем), и что еще более важно, условно-модифицируемых причин (недостаток кофакторов, избыточное потребление метионина, прием лекарственных препаратов) для своевременного их нивелирования. Также представляется важным изучение подходов к коррекции ГГЦ на прегравидарном этапе реализации репродуктивной функции женщины для формирования корректной «программы» развития плода и новорожденного.</p></abstract><trans-abstract xml:lang="en"><p>Today, a whole body of evidence base has been formed showing the negative effect of hyperhomocysteinemia (HHC) during pregnancy on the intrauterine fetal development: risk factors associated with development of congenital neural tube defects, congenital heart defects and non-syndromic oral facial clefts. In addition, numerous studies have shown that HHC is a risk factor for serious pregnancy complications such as repeated pregnancy loss, preterm birth, preeclampsia, placental abruption, intrauterine growth restriction, gestational diabetes mellitus. At the same time, despite multiple clinical and experimental studies, the significance of HHC effects continues to support the interest to this issue. In recent years, new data were revealed about littleknown phenomena pregnancy-associated maternal HHC such as "epigenetic fetal aging" and "fetal atherogenesis". Epigenetic aging is an individual indicator of aging that captures interindividual differences at disease onset throughout life occurring during intrauterine development being reveaked as DNA hypomethylation. Moderate HHC during pregnancy causes fetal DNA hypomethylation, which, according to recent research, may represent the main mechanism underlying health in childhood and adulthood. "Fetal atherogenesis" is another understudied HHC effect emphasizing that the formation of atherosclerotic plaques begins not in adulthood, but in utero. A body of studies has shown that the formation of thickening and fatty streaks in vascular intima involved in the pathogenesis of atherosclerosis begins antenatally. The discovery of new aspects of negative intrauterine HHC impact on fetal development necessitates a detailed study of HHC causes as non-modifiable (genetic defects in enzyme systems), and more importantly, conditionally modifiable causes (lack of cofactors, excessive methionine consumption, medications) for their timely leveling. It is also important to study approaches to HHC correction at the preconception stage of a female reproductive function in order to correctly "program" fetus and newborn development.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>гипергомоцистеинемия</kwd><kwd>ГГЦ</kwd><kwd>акушерские осложнения</kwd><kwd>«плодовый атерогенез»</kwd><kwd>«эпигенетическое старение плода»</kwd></kwd-group><kwd-group xml:lang="en"><kwd>hyperhomocysteinemia</kwd><kwd>HHC</kwd><kwd>pregnancy complications</kwd><kwd>"fetal atherogenesis"</kwd><kwd>"epigenetic fetal aging"</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Работа подготовлена в рамках выполнения поисковой научной темы – государственного задания Министерства науки и высшего образования Российской Федерации № 1022040700837-4-3.2.2-1-11 «Разработка технологии раннего прогнозирования и профилактики врожденных пороков развития плода и перинатальной патологии на основе оценки нутриентного статуса, биохимических и молекулярно-генетических маркеров на этапе прегравидарной подготовки и при беременности».</funding-statement><funding-statement xml:lang="en">The study was prepared within the framework of the research topic – state assignment of the Ministry of Science and Higher Education of the Russian Federation No. 1022040700837-4-3.2.2-1-11 "Development of technology for early prediction and prevention of fetal congenital malformations and perinatal pathology based on assessed nutritional status, biochemical and molecular genetic markers at the stage of preconception preparation and pregnancy".</funding-statement></funding-group></article-meta></front><body><sec><title>Введение / Introduction</title><p>Фетальное программирование – это направление перинатологии, которое изучает все возможные негативные и позитивные факторы, воздействующие в период внутриутробного развития на эмбрион и плод.</p><p>Гипергомоцистеинемия (ГГЦ) при беременности создает определенный «микроклимат» для плода, реализующий ряд негативных эффектов на разных сроках гестации и определяющий его приверженность ко многим заболеваниям во взрослом возрасте.</p><p>На ранних сроках беременности, когда происходит закладка органов, умеренная ГГЦ повышает риск врожденных пороков развития, таких как дефекты нервной трубки, врожденные пороки сердца, несиндромные расщелины полости рта [<xref ref-type="bibr" rid="cit1">1</xref>].</p><p>Последние данные указывают на критический период поздних сроков беременности и негативное внутриутробное влияние гомоцистеина (ГЦ) на сосуды плода, что ассоциировано с ранними признаками атеросклероза у детей. Было показано, что более высокие концентрации ГЦ в пуповинной крови, взятые при рождении, были связаны с меньшей растяжимостью сонных артерий в школьном возрасте [<xref ref-type="bibr" rid="cit2">2</xref>]. Так появилась идея о «плодовом атерогенезе».</p><p>Гипергомоцистеинемия влияет на эпигенетическую регуляцию экспрессии генов посредством вмешательства в метаболизм метильной группы, вызывая при этом гипометилирование ДНК, что обуславливает «эпигенетическое старение плода». Разность между эпигенетическим и хронологическим возрастом называется возрастным ускорением. Положительное возрастное ускорение указывает на более старший эпигенетический возраст. У взрослых положительное ускорение возраста связано со смертностью от всех причин, а также со смертностью от рака и сердечно-сосудистых заболеваний [3–5].</p><p>Изменения в метилировании ДНК плода могут быть основным механизмом, связанным с последствиями для здоровья в детском и взрослом возрасте [<xref ref-type="bibr" rid="cit6">6</xref>]. Были найдены некоторые доказательства, подтверждающие связь умеренной ГГЦ матери с положительным ускорением гестационного возраста плода, что свидетельствует о более быстром эпигенетическом, чем клиническом гестационном старении [<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Умеренная ГГЦ – это многофакторное состояние, обусловленное наследственностью и образом жизни, которое может быть скорректировано даже у наследственно предрасположенных женщин. Сегодня основные подходы к коррекции – это применение препаратов фолиевой кислоты, витаминов B6, В12 и витамина D.</p></sec><sec><title>Гомоцистеин и гипергомоцистеинемия / Homocysteine and hyperhomocysteinemia</title><p>Гипергомоцистеинемия подразделяется на тяжелую (&gt; 100 мкмоль/л), промежуточную (31–100 мкмоль/л) и умеренную (16–30 мкмоль/л) формы. Промежуточная и умеренная ГГЦ не имеют клинических признаков, наблюдаемых при тяжелой ГГЦ, за исключением положительной ассоциации с развитием окклюзионного заболевания сосудов [<xref ref-type="bibr" rid="cit7">7</xref>].</p><p>В 90-х гг. ХХ века концентрация ГЦ в 15 мкмоль/л общепринято считалась верхней границей нормального уровня ГЦ [8–13]. Однако впоследствии стало известно, что риск развития сосудистой патологии увеличивается при концентрации ГЦ свыше 10 мкмоль/л [<xref ref-type="bibr" rid="cit14">14</xref>].</p><p>Данные ФГБУ «Российский научно-исследовательский институт гематологии и трансфузиологии Федерального медико-биологического агентства» (ФГБУ РосНИИГТ ФМБА России) приводят следующие возрастные нормы ГЦ: вне беременности для женщин ≤ 30 лет – 4,6–8,1 мкмоль/л, для женщин старше 30 лет – 4,5–8,0 мкмоль/л и для мужчин – 4,6–11,2 мкмоль/л [<xref ref-type="bibr" rid="cit15">15</xref>]. По данным литературы, значение ГЦ у мужчин в среднем на 2–4 мкмоль/л выше, чем у женщин [<xref ref-type="bibr" rid="cit16">16</xref>].</p><p>Однако, несмотря на многолетнее изучение ГЦ, до сих пор остается открытым вопрос о его оптимальном уровне у женщин в репродуктивном возрасте с точки зрения акушерского взгляда.</p><p>В исследовании K. Rasmussen с соавт. концентрация ГЦ 4,5–7,9 мкмоль/л стала оптимальной для женщин репродуктивного возраста [<xref ref-type="bibr" rid="cit17">17</xref>].</p><p>Если уровень ГЦ больше 7,9 мкмоль/л, то риск невынашивания беременности увеличивается в 2,2 раза (отношение шансов (ОШ) = 2,2; 95 % доверительный интервал (ДИ) = 1,4–3,5) [<xref ref-type="bibr" rid="cit18">18</xref>].</p><p>По сравнению с небеременными значения ГЦ при нормальной беременности ниже. Более низкий уровень ГЦ обусловлен гемодилюцией, вызванной увеличением объема крови, повышенной скоростью клубочковой фильтрации и потреблением части ГЦ плодом [<xref ref-type="bibr" rid="cit19">19</xref>]. На уровень ГЦ во время беременности также может влиять гормональная среда. Концентрация ГЦ ниже у женщин в пременопаузе, чем в постменопаузе [20–22].</p><p>Концентрация ГЦ в постменопаузе снижалась после заместительной гормональной терапии. Содержание эстрогена линейно возрастало в сыворотке крови матери во время беременности с 92 до 642 пмоль/л, диапазон II триместра составлял 275–367 пмоль/л [<xref ref-type="bibr" rid="cit23">23</xref>]. Показано, что гормональная среда во время беременности может способствовать снижению уровня ГЦ в плазме [<xref ref-type="bibr" rid="cit24">24</xref>].</p><p>Уровень ГЦ снижается на ранних сроках беременности, достигает своего самого низкого значения во II триместре, после этого неуклонно нарастает на поздних сроках, пока не достигнет уровня ранних сроков [<xref ref-type="bibr" rid="cit25">25</xref>]. Однако существуют различия в популяциях: так, среди беременных азиатской популяции содержание ГЦ уменьшается на ранних сроках и неуклонно растет во II и III триместрах [<xref ref-type="bibr" rid="cit26">26</xref>].</p><p>Нормальные значения ГЦ во время беременности следующие: 3,9–7,3 ммоль/л до 16-й недели гестации, 3,5–5,3 ммоль/л между 20-й и 24-й неделями и 3,3–7,5 ммоль/л после 36-й недели [<xref ref-type="bibr" rid="cit25">25</xref>]. В таблице 1 приведены уровни ГЦ в разных триместрах нормальной беременности. Несмотря на то что нормальное значение ГЦ на поздних сроках беременности меньше, чем до беременности, беременные более восприимчивы к ГГЦ [<xref ref-type="bibr" rid="cit25">25</xref>][<xref ref-type="bibr" rid="cit27">27</xref>], так как ГГЦ снижает кровообращение на границе раздела мать–плод и уменьшает перфузию плаценты [<xref ref-type="bibr" rid="cit25">25</xref>].</p><table-wrap id="table-1"><caption><p>Таблица 1. Из-за своего тиолового компонента ГЦ может генерировать высокореактивные восстановленные формы кислорода, которые значительно повреждают клеточные структуры. В совокупности это известно как окислительный стресс.</p><p>В основном, метаболизм ГЦ протекает в печени, меньшая его часть метаболизируется через почки. В организм человека ГЦ попадает в виде метионина с белками животного происхождения. Метионин в свою очередь поступает в цикл метилирования, в результате которого образуются ГЦ и SAM (S-аденозилметионин) – универсальный метильный донор. Цикл метионин-гомоцистеин является источником одноуглеродных фрагментов (метильных групп -СН3), использующихся для метилирования ДНК, РНК, белков, фосфолипидов и синтеза нуклеотидов.</p><p>Основным источником ГЦ в плазме крови являются пролиферирующие клетки, так как рост и регенерация тканей резко увеличивают потребность клеток в метильных группах, используемых для синтеза нуклеотидов. Почти весь ГЦ метаболизируется внутри клеток, и лишь малая его часть попадает в кровоток.</p><p>В плазме крови примерно 80 % ГЦ находится в связанном с альбумином состоянии, около 1 % – в свободной форме. Остальная часть присутствует в виде дисульфидов с цистеином, гомоцистеином (гомоцистин) и другими соединениями. Все гомоцистеин-содержащие производные определяются как «общий гомоцистеин» (англ. total homocysteine, tHcy) плазмы крови.</p><p>Гомоцистеин метаболизируется двумя основными путями: первый путь – реметилирования, второй – транссульфирования. Эти механизмы поддерживают концентрацию ГЦ на относительно постоянном уровне в плазме крови.</p><p>Цианокобаламин (витамин B12) регулирует путь реметилирования при участии фермента метионин-синтазы (MTR); данный путь тесно сопряжен с фолатным циклом. Биологически активная форма фолиевой кислоты – 5-метилтетрагидрофолат при участии фермента метилентетрагидрофолатредуктазы (MTHFR) отдает метильную группу витамину B12 и восстанавливается до тетрагидрофолата. С помощью фермента MTR метильная группа передается ГЦ, таким образом образуется метионин.</p><p>Существует альтернативный вариант метилирования ГЦ, так называемая нецианокобалазависимая трансформация, которая в основном происходит в печени и регулируется количеством холина. Данный путь протекает с участием фермента – бетаин-гомоцистеин-метилтрансферазы (англ. betaine homocysteine methyltransferase, BНMT). Так как последняя реакция не встречается в головном мозге, ГГЦ в нервной системе может развиваться быстрее, чем в других органах и тканях.</p><p>Цистатионин-β-синтаза (англ. cystathionine-β-synthase, CBS), пиридоксаль-5-фосфат зависимый фермент, катализирует реакцию транссульфирования с образованием цистеина, который в свою очередь окисляется до таурина и неорганических сульфатов, востребованных в синтезе прямых антикоагулянтов – гепарина, гепаран-сульфата, а также хондроитин сульфата.</p><p>Таким образом, для утилизации ГЦ используются 4 основных фермента: MTR, MTHFR, BНMT, CBS, а также витамины B6, B9 и B12 [<xref ref-type="bibr" rid="cit25">25</xref>].</p></sec><sec><title>Причины гипергомоцистеинемии / Causes of hyperhomocysteinemia</title><p>Гипергомоцистеинемия – это многофакторное состояние, причины которого делят на врожденные (генетические дефекты ферментных систем) и приобретенные (недостаток кофакторов, избыточное потребление метионина, некоторые заболевания и прием определенных лекарств) [29–32]</p><p>Как правило, умеренная ГГЦ – это сочетание нескольких факторов, одним из которых может быть наследственная составляющая. Благодаря модифицируемым факторам, у женщин с умеренной ГГЦ возможна коррекция уровня ГЦ в плазме крови до оптимального.</p><p>Генетические дефекты ферментных систем / Genetic defects in enzyme pathways</p><p>Наиболее подробно изучены гены CBS, MTHFR и метионин-синтазы (MS).</p><p>Дефекты в гене CBS приводят к дефициту активности цистатион-β-синтазы – ключевого фермента метаболизма органической серы и редкой врожденной патологии, называемой классической гомоцистеинурией (ГЦУ). При этом состоянии концентрация общего ГЦ превышает 100 мкмоль/л. ГЦУ относится к аутосомно-рецессивным заболеваниям [<xref ref-type="bibr" rid="cit33">33</xref>]. Ген CBS расположен в субтеломерной области хромосомы 21q22.3 [<xref ref-type="bibr" rid="cit34">34</xref>].</p><p>Интересный факт: в 1977 г. было показано, что сосуды пациентов с болезнью Дауна не склонны к образованию атеросклероза. Данное явление объясняется наличием трех копий хромосомы 21, что обуславливает повышенную активность CBS [<xref ref-type="bibr" rid="cit35">35</xref>].</p><p>База данных Сlinically relevant variation (СlinVar) содержит информацию о 894 вариантах полиморфизмов гена CBS, 67 из которых отнесены к патогенным, 29 – к вероятно патогенным [<xref ref-type="bibr" rid="cit36">36</xref>]. Примеры патогенных полиморфизмов CBS, ведущих к классической ГЦУ, следующие: c.1330G&gt;A (p.Asp444Asn); c.1152G&gt;C (p.Lys384Asn); c.1150A&gt;G (p.Lys384Glu) – чувствителен к пиридоксину.</p><p>Симптомы заболевания манифестируют на первом году жизни и проявляются как от легких, так и до тяжелых форм. ГЦУ, обычно диагностируемая в детском или подростковом возрасте, проявляется тромбоэмболией, эктопией хрусталика, остеопорозом, умственной отсталостью и другими неврологическими и психиатрическими отклонениями [<xref ref-type="bibr" rid="cit37">37</xref>]. Выделяют B6-чувствительную и B6-резистентную формы. B6-чувствительная форма имеет более мягкий фенотип и более позднее начало. При B6-резистентной форме клиническая картина манифистирует раньше, и проявления болезни более тяжелые [<xref ref-type="bibr" rid="cit39">39</xref>].</p><p>CBS человека экспрессируется в печени, почках, мышцах, головном мозге и яичниках, а также во время раннего эмбриогенеза в нервной и сердечной системах, что и обуславливает более агрессивное и раннее проявление болезни [<xref ref-type="bibr" rid="cit39">39</xref>].</p><p>Однако не все полиморфизмы CBS приводят к ГГЦ; например, было обнаружено, что c.699C&gt;T и c.1080C&gt;T связаны со снижением риска ишемической болезни сердца и с повышенной чувствительностью к эффектам фолиевой кислоты, снижающим уровень ГЦ [<xref ref-type="bibr" rid="cit40">40</xref>].</p><p>Гены MTHFR и MS способствуют реметилированию ГЦ. В базе данных СlinVar представлено 698 вариаций однонуклеотидных полиморфизмов в гене MTHFR; из них – 120 патогенных и 50 – вероятно патогенных [<xref ref-type="bibr" rid="cit36">36</xref>]. Патогенные варианты являются причинами ГГЦ и ГЦУ. Клинические проявления варьируют от тяжелых до легких форм в зависимости от конкретных SNP-полиморфизмов (англ. Single Nucleotide Polymorphism) или их сочетаний.</p><p>Примеры однонуклеотидных патогенных полиморфизмов MTHFR: c.1808C&gt;G (с.Ser603Cys); c.1883T&gt;C (p.Leu628Pro); c.1530G&gt;A (p.Lys510 =).</p><p>Cогласно клиническим рекомендациям «Нарушение обмена серосодержащих аминокислот» от 2022 г., ГЦУ, обусловленная дефицитом MTHFR, имеет следующие проявления [<xref ref-type="bibr" rid="cit41">41</xref>]:</p><p>Тяжелые случаи ГГЦ, в основном, обусловлены дефектами в генах CBS и MTHFR и требуют немедленной коррекции. При отсутствии лечения тяжелой ГГЦ дети с такими генетическими дефектами умирают от осложнений каротидного атеросклероза, тромбоза и инфаркта головного мозга в раннем возрасте [42–44].</p><p>Умеренное повышение уровня ГЦ – это риск сердечно-сосудистых заболеваний. По данным многолетних ретроспективных и проспективных наблюдательных исследований на людях было установлено, что ГГЦ является мощным независимым фактором риска развития атеросклероза [<xref ref-type="bibr" rid="cit34">34</xref>]. Данные открытия привлекли внимание сообщества акушеров-гинекологов по всему миру, так как сосудистые заболевания – это риск многих акушерских осложнений и патологического влияния на плод. Сегодня с ГГЦ связывают следующие осложнения беременности: привычное невынашивание, преэклампсия, преждевременная отслойка плаценты, преждевременные роды, задержка развития плода, дефекты развития нервной трубки и гестационный сахарный диабет [<xref ref-type="bibr" rid="cit15">15</xref>][<xref ref-type="bibr" rid="cit18">18</xref>][45–53]. Соответственно, усилилось изучение детерминант умеренной ГГЦ.</p><p>Установлено, что умеренную ГГЦ чаще вызывают полиморфизмы в гене MTHFR [54–56].</p><p>В 1988 г. S.S. Kang с соавт. сообщили о варианте фермента N(5,10)MTHFR, который отличался от нормального фермента своей более низкой активностью и чувствительностью к теплу [<xref ref-type="bibr" rid="cit54">54</xref>]. Впоследствии выяснили, что термолабильный N(5,10)MTHFR вызывается миссенс-мутацией аланин-валин [<xref ref-type="bibr" rid="cit55">55</xref>].</p><p>В 1991 г. S.S. Kang с соавт. продемонстрировали, что уровни ГЦ в плазме были выше у лиц с термолабильным MTHFR [<xref ref-type="bibr" rid="cit56">56</xref>]; однако у некоторых людей, у которых был обнаружен термолабильный фермент, не было выявлено ГГЦ. Данное несовпадение обуславливали низкие концентрации фолатов [<xref ref-type="bibr" rid="cit57">57</xref>]. Таким образом, фолатный статус играет решающую роль в развитии умеренной ГГЦ у лиц с термолабильным ферментом.</p><p>Два наиболее изученных полиморфизма в гене MTHFR – c.665C&gt;T и c.1286A&gt;C являются причинами умеренной ГГЦ в условиях дефицита фолиевой кислоты [<xref ref-type="bibr" rid="cit57">57</xref>][<xref ref-type="bibr" rid="cit58">58</xref>], именно поэтому данные полиморфизмы являются доброкачественными.</p><p>Показано, что среди лиц с уровнем фолата в плазме ниже медианы выборки (15,4 нмоль/л) средняя концентрация ГЦ натощак у гомозигот с термолабильной мутацией была на 24 % выше (р &lt; 0,05), чем у нормальных или гетерозиготных индивидуумов. Также не наблюдалось никакой связи между генотипом и концентрацией ГЦ натощак у лиц с содержанием фолата в плазме на уровне медианы или выше [<xref ref-type="bibr" rid="cit57">57</xref>].</p><p>Другие причины гипергомоцистеинемии / Other causes of hyperhomocysteinemia</p><p>Уровень ГЦ плазмы увеличивается и у мужчин и у женщин с возрастом, при этом у женщин он ассоциирован с менопаузой. Образ жизни, а именно, кофеин, курение, сидячий образ жизни, избыточное употребление алкоголя повышают концентрацию ГЦ. Однако недостаток фолиевой кислоты, витаминов В12, В6 и витамина D, хронические колиты, почечная недостаточность, гипофункция щитовидной железы и пересадка органов увеличивают уровень ГЦ плазмы.</p><p>К лекарственным средствам, влияющим на уровень ГЦ, относятся метотрексат и карбамазепин – ингибиторы фолатов, также холестипол или холестирамин, которые являются антагонистами абсорбции фолатов в кишке. К дефициту витамина В6 приводит прием ниацина или теофиллина. Как антагонист кобаламина действует оксид азота. Почечный метаболизм нарушает прием циклоспоринов, что ведет к ГГЦ. Прием метформина в отсутствие экзогенных добавок фолиевой кислоты или витаминов группы В достоверно повышает уровень ГЦ.</p></sec><sec><title>Механизм повреждения клеток при гипергомоцистеинемии / Mechanism of cell damage in hyperhomocysteinemia</title><p>Гипергомоцистеинемия индуцирует окислительный стресс, эндотелиальную дисфункцию, воспаление, пролиферацию гладкомышечных клеток и стресс эндоплазматического ретикулума [<xref ref-type="bibr" rid="cit59">59</xref>].</p><p>Эндотелиальная дисфункция, определяемая как нарушение эндотелийзависимого расслабления кровеносных сосудов, является характерной чертой многих сердечно-сосудистых факторов риска, включая гипертонию, гиперхолестеринемию и диабет [<xref ref-type="bibr" rid="cit60">60</xref>][<xref ref-type="bibr" rid="cit61">61</xref>]. Нарушение вазомоторной функции эндотелия при ГГЦ наблюдалось в экспериментальных моделях у обезьян [<xref ref-type="bibr" rid="cit62">62</xref>], мини-свиней [<xref ref-type="bibr" rid="cit63">63</xref>], крыс [<xref ref-type="bibr" rid="cit64">64</xref>] и мышей [<xref ref-type="bibr" rid="cit65">65</xref>][<xref ref-type="bibr" rid="cit66">66</xref>]. Аномальная эндотелийзависимая вазорелаксация также возникает у людей с острой ГГЦ, вызванной пероральной нагрузкой метионином [<xref ref-type="bibr" rid="cit67">67</xref>][<xref ref-type="bibr" rid="cit68">68</xref>]. Степень нарушения функции эндотелия при ГГЦ аналогична той, которая наблюдается при других факторах риска, таких как гиперхолестеринемия и гипертония [<xref ref-type="bibr" rid="cit59">59</xref>].</p><p>Основной механизм индуцированной ГГЦ эндотелиальной дисфункции, по-видимому, связан со снижением биодоступности оксида азота (NO), получаемого из эндотелия [<xref ref-type="bibr" rid="cit69">69</xref>]. Причина нарушения биодоступности NO при ГГЦ, вероятно, многофакторна [<xref ref-type="bibr" rid="cit69">69</xref>], но большая часть данных, полученных на животных моделях, свидетельствует о том, что гомоцистеин-индуцированный окислительный стресс, приводящий к ускоренной окислительной инактивации NO, является основным механизмом.</p><p>Высокореактивная тиоловая группа гомоцистеина легко окисляется с образованием активных форм кислорода, включая супероксид анион, перекись водорода и гидроксильный радикал. Исследования in vitro продемонстрировали, что ГЦ может стимулировать образование анионов супероксида и пероксинитрита, которые индуцировали окисление сульфгидрильных групп в эндотелиальной синтазе окиси азота (англ. endothelial nitric oxide synthase, eNOS). Данный процесс приводит к снижению NO-генерирующей активности фермента [<xref ref-type="bibr" rid="cit70">70</xref>]. Также сообщалась, что пероксинитрит инициирует апоптоз in vitro посредством активации митоген-активируемых протеинкиназ (англ. mitogen-activated protein kinases, MAPK) и c-Jun N-концевых киназ (англ. c-Jun N-terminal kinases, JNKs) [<xref ref-type="bibr" rid="cit71">71</xref>]. Показано, что ГГЦ ингибирует антиоксидантную активность клеток, снижая экспрессию гемоксигеназы-1 (англ. heme oxygenase-1, HO-1) и глутатионпероксидазы (англ. glutathione peroxidase, GPx) – основных ферментов, защищающих клетки от окислительного повреждения [<xref ref-type="bibr" rid="cit72">72</xref>].</p><p>Гипергомоцистеинемия коррелирует с повышенным уровнем воспалительных цитокинов, хемокинов, молекул адгезии лейкоцитов, которые способствуют воспалению сосудов и атерогенезу [<xref ref-type="bibr" rid="cit73">73</xref>].</p><p>Одно из наиболее интересных клеточных последствий, индуцированных ГЦ, происходит в эндоплазматическом ретикулуме (ЭР). Когда внутриклеточное содержание ГЦ повышается, он может участвовать в реакциях дисульфидного обмена с белками ЭР, что приводит к неправильному свертыванию вновь синтезированных секреторных и мембранных белков, таких как тромбомодулин и фактор фон Виллебранда (англ. von Willebrand factor, vWF) [74, 75]. Клеточные последствия этого состояния, известного как стресс ЭР, включают нарушение регуляции липидного обмена, активацию воспалительных путей и нарушение передачи сигналов инсулина [<xref ref-type="bibr" rid="cit76">76</xref>]. Тяжелый или длительный стресс ЭР может привести к апоптотической гибели клеток [<xref ref-type="bibr" rid="cit77">77</xref>].</p><p>Гомоцистеин в умеренных и высоких концентрациях конкурирует с S-аденозилметионином (SАМ) за сайты связывания на ДНК-метилтрансферазах и может стать причиной пассивной потери метилирования в реплицирующейся ДНК. Гипометилирование является характерной чертой процесса старения организма (тотальное гипометилирование генома) и фактором образования опухолевых клеток, поскольку связано с развитием хромосомной нестабильности.</p><p>Повышенный уровень ГЦ отрицательно влияет на репродуктивную функцию, что объясняется его воздействием на микротромбообразование и нарушение микроциркуляции. Данный механизм играет значимую роль в развитии акушерских осложнений, связанных с изменением маточно-плацентарного кровообращения [<xref ref-type="bibr" rid="cit77">77</xref>].</p></sec><sec><title>Малоизученные эффекты умеренной гипергомоцистеинемии: влияние на плод / Under-studied effects of moderate hyperhomocysteinemia: effects on the fetus</title><p>Атеросклероз и связанные с ним сердечно-сосудистые заболевания, по-видимому, возникают частично в раннем детстве, и даже у плода [78–80].</p><p>Образование утолщений и жировых прожилок интимы сосудов, участвующих в патогенезе атеросклероза, начинается внутриутробно. Увеличенная толщина интимы-медиа сонной артерии и меньшая ее растяжимость рассматриваются как ранние признаки атеросклероза [81–83].</p><p>Сегодня известно, что питание и рост плода могут быть критическими факторами атерогенеза [<xref ref-type="bibr" rid="cit84">84</xref>].</p><p>Данной теме посвящено исследование «Поколения R» – многоцентровое популяционное проспективное когортное исследование, которое проводит поиск ранних экологических, эндокринных, генетических, эпигенетических, микробиомных факторов, воздействующих на плод и ведущих к проявлению болезней в детстве и во взрослой жизни [<xref ref-type="bibr" rid="cit85">85</xref>]. В исследовании фокусируется внимание на нескольких аспектах здоровья: поведение, познание, иммунитет, развитие тела, органов зрения, слуха, полости рта, лица, сердечно-сосудистой и респираторной систем, инфекционные, аллергические и кожные заболевания. В общей сложности в когорту было включено 9778 матерей с датой родов с апреля 2002 г. по январь 2006 г. Сегодня детям «Поколения R» около 20 лет, и они до сих пор продолжают участвовать в исследовании. Некоторые результаты данного исследования уже освещены в публикациях.</p><p>Было показано, что более высокие концентрации ГЦ в пуповинной крови, взятые при рождении, были связаны с меньшей растяжимостью сонных артерий в школьном возрасте. В роттердамском исследовании среди 3826 пар мать–ребенок была изучена ассоциация маркеров одноуглеродного обмена с толщиной и растяжимостью интима-медиа сонной артерии у детей в возрасте 10 лет. Содержание общего и активного витамина B12, фолиевой кислоты и ГЦ оценивали на ранних сроках беременности и в пуповинной крови при рождении [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Результаты показали, что низкие общие концентрации B12 у матерей на ранних сроках беременности связаны с более высокой толщиной интима-медиа сонной артерии у детей школьного возраста, а низкие концентрации фолата на ранних сроках беременности, по-видимому, связаны с более низкой растяжимостью сонной артерии. Более высокие концентрации ГЦ пуповинной крови, отобранные при рождении, были связаны с более низкой растяжимостью сонной артерии.</p><p>Данные указывают на критический период поздних сроков беременности и негативное внутриутробное влияние ГЦ на сосуды плода. Данное исследование является первым, в котором оценивалась связь одноуглеродных маркеров метаболизма во время беременности с ранними признаками атеросклероза у детей [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Известен еще один анализ когорты «Поколение R», где оценивали связь циркулирующих концентраций фолата, витамина B12 и ГЦ с кардиометаболическими показателями у детей в возрасте 10 лет [<xref ref-type="bibr" rid="cit86">86</xref>]. Отбор проб крови проводили во время беременности и при рождении из пуповинной крови. В данном исследовании приняло участие 4449 пар мать–ребенок. К кардиометаболическим показателям относятся: индекс массы тела (ИМТ), распределение жира в организме, частота сердечных сокращений, артериальное давление, содержание инсулина, глюкозы и липидов. Стало известно, что более высокие концентрации фолата в плазме матери были связаны с более низким ИМТ, соотношением андроид-гиноиднового жира, систолическим артериальным давлением, риском избыточной массы тела и кластеризацией сердечно-сосудистых факторов риска. Более высокая общая концентрация B12 в материнской сыворотке была связана с более низким уровнем глюкозы и более высокими концентрациями липопротеинов высокой плотности.</p><p>Интересно, что концентрации маркеров одноуглеродного обмена в пуповинной крови не были связаны с кардиометаболическими показателями у детей 10 лет. Также в данном исследовании не выявлено никаких причинно-следственных связей между концентрацией ГЦ с кардиометаболическими показателями у детей [<xref ref-type="bibr" rid="cit86">86</xref>].</p><p>Одной из основных целей геронаучных исследований является определение биомаркеров старения [<xref ref-type="bibr" rid="cit87">87</xref>][<xref ref-type="bibr" rid="cit88">88</xref>]. А одним из потенциальных биомаркеров, к которому в последние годы проявился значительный интерес, является метилирование ДНК (DNAm), которое определяет биологический возраст человека. Было показано, что хронологическое время вызывает предсказуемые изменения гипо- и гиперметилирования во многих областях генома [89–93].</p><p>Разность между эпигенетическим и хронологическим возрастом называется возрастным ускорением. Положительное возрастное ускорение указывает на более старший эпигенетический возраст; отрицательное возрастное ускорение указывает на более старший хронологический возраст. У взрослых положительное ускорение возраста было связано со смертностью от всех причин, а также со смертностью от рака и сердечно-сосудистых заболеваний [3–5].</p><p>Интересно, что положительное возрастное ускорение имеет место и во внутриутробном развитии. Данное явление можно рассматривать как индивидуальный показатель старения, который фиксирует межиндивидуальные различия во времени начала какого-либо заболевания, функционального снижения и смерти на протяжении всей жизни [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Недавно на основе данных метилирования ДНК новорожденных были разработаны 2 эпигенетических часа для гестационного возраста [<xref ref-type="bibr" rid="cit94">94</xref>][<xref ref-type="bibr" rid="cit95">95</xref>].</p><p>Изменения в метилировании ДНК плода могут быть основным механизмом, связанным с последствиями для здоровья в детском возрасте [<xref ref-type="bibr" rid="cit6">6</xref>]. Существуют популяционные исследования, которые установили взаимосвязь между моделями метилирования ДНК, нейроразвитием и степенью ожирения в постнатальном периоде [<xref ref-type="bibr" rid="cit96">96</xref>].</p><p>Были найдены некоторые доказательства, подтверждающие связь более высокой концентрации ГЦ (≥ 19 мкмоль/л) в плазме крови матери с положительным ускорением гестационного возраста плода (0,07 недели; 95 % ДИ = 0,02–0,13), что свидетельствует о более быстром эпигенетическом, чем клиническом гестационном старении [<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Фолиевая кислота, витамин В12 и ГЦ являются частью одноуглеродного метаболизма, которые влияют на клеточный рост, дифференцировку, а также биосинтез, репарацию и метилирование ДНК [<xref ref-type="bibr" rid="cit97">97</xref>].</p><p>Внутриутробное положительное ускорение гестационного возраста негативно влияет на нейроразвитие и степень ожирения в детском возрасте [<xref ref-type="bibr" rid="cit6">6</xref>][<xref ref-type="bibr" rid="cit59">59</xref>]. Концентрация витамина В12 (≥ 145 пмоль/л) в сыворотке крови пуповины была связана с отрицательным ускорением гестационного возраста.</p></sec><sec><title>Современные подходы к коррекции гипергомоцистеинемии / Current approaches for correction of hyperhomocysteinemia</title><p>Назначение адекватных профилактических или лечебных доз фолиевой кислоты, витаминов B6 и В12 до и во время беременности является основным патогенетическим лечением ГГЦ. Диета, богатая фолиевой кислотой (1 мг/сут), витаминами B6 (12 мг/сут) и B12 (50 мкг/сут) целесообразна в качестве профилактики только в случае исходного оптимального уровня ГЦ в плазме крови.</p><p>Фолиевая кислота представляет собой синтетическую форму фолата, которая должна быть восстановлена дигидрофолатредуктазой в дигидрофолат, а затем в тетрагидрофолат посредством ферментативного восстановления. Последние данные свидетельствуют о том, что максимальная способность к этому процессу ограничена, и в кровообращении обнаружена неметаболизированная фолиевая кислота. Биологические эффекты неметаболизированной фолиевой кислоты неизвестны.</p><p>Метафолин – препарат, созданный на основе 5-метил-тетрагидрофолата. Метафолин в отличие от фолиевой кислоты непосредственно поступает в организм в биологически активной форме. Препарат эффективен независимо от наличия у пациента гомозиготного и/или гетерозиготного генотипов 677C→T полиморфизма MTHFR. В связи с этим с целью коррекции фолатного статуса во время беременности метафолин может стать полной альтернативой, если эффективность данного препарата будет доказана [<xref ref-type="bibr" rid="cit98">98</xref>].</p><p>Последние исследования показывают, что нормальный уровень витамина D может снизить патологическое воздействие ГГЦ на сосуды. Однако данная гипотеза нуждается в дальнейшем подтверждении в более крупных рандомизированных исследованиях [<xref ref-type="bibr" rid="cit99">99</xref>].</p><p>Было обнаружено, что существует обратная зависимость между 25(OH)D и ГЦ среди лиц с уровнем 25(OH)D ≤ 21 нг/мл. Но не наблюдалось какого-либо статистического снижения ГЦ, когда концентрация 25(OH)D поднималась выше 21 нг/мл [<xref ref-type="bibr" rid="cit100">100</xref>].</p></sec><sec><title>Заключение / Conclusion</title><p>В данном обзоре приведенная информация о роли ГЦ при беременности свидетельствует о важности своевременной коррекции его уровня. Эффекты умеренной ГГЦ замаскированы и незаметны, однако способны определять восприимчивость взрослого человека к хроническим и острым заболеваниям. Поэтому о вопросе «фетального программирования» важно информировать женщин на прегравидарном этапе реализации репродуктивной функции.</p><p>Своевременная коррекция умеренной ГГЦ снижает риски акушерских осложнений (привычное невынашивание, преэклампсия, преждевременная отслойка плаценты, преждевременные роды, задержка развития плода и гестационный сахарный диабет), врожденных пороков развития (дефекты нервной трубки, врожденные пороки сердца, несиндромные расщелины полости рта), а также негативного влияния на здоровье в детском и взрослом возрасте (внутриутробного «старения» плода и «плодового атерогенеза»).</p><p>Препараты фолиевой кислоты, витамины B6, B12 и D являются основным подходом к терапии ГГЦ.
