Prior to the first visit to our clinic, the woman was never examined for inherited or acquired forms of thrombophilia, although her medical and family history was indicative of a hereditary element of the disease. Here, homozygous mutation of prothrombin factor II (Thr165Met) and polymorphisms of the folate cycle and platelet receptor genes were detected. Managing of pregnancy, delivery and the postpartum period included carefully selected doses of low-molecular weight heparin under continuous monitoring of hemostasis and heparin dose correction.</p></abstract><trans-abstract xml:lang="ru"><p>В статье описан клинический случай планирования, ведения беременности и родов у женщины высокого риска развития тромботических и акушерских осложнений в связи с перенесенной массивной двусторонней рецидивирующей тромбоэмболией легочной артерии, инфарктной пневмонией и тромбозом плечевой и подключичной вен в анамнезе на фоне приема гормональных контрацептивов. На момент обращения в нашу клинику женщина не была обследована на наличие генетических и приобретенных форм тромбофилии, хотя ее тромботический и семейный анамнез указывали на наличие наследственного характера заболевания. У нее была выявлена гомозиготная мутация фактора II протромбина (Thr165Met), а также полиморфизмы генов фолатного цикла и тромбоцитарных рецепторов.
