Women with infertility and circulating antiphospholipid  antibodies  (APA) may be at risk of reproductive losses. A possible way to improve their in vitro fertilization  (IVF) outcomes is the immunomodulating  therapy using intravenous immunoglobulin (IVIG). Aim: to study the effecacy of IVIG in women with APA who undergo IVF (IVF/ICSI).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. One hundred twenty eight APA-positive women were enrolled: 77 women were  given  IVIG during the IVF procedure at a course dose of 15 g (group 1), 55 women received no IVIG (group  2) and 63 women without APA scheduled for IVF made up the control group (group 3). Women in groups 1 and 2 received prophylactic doses of low weight heparins and reduced doses of acetylsalicylic acid. Women with poor quality embryos were excluded. The implantation rate (positive serum β-HCG), the clinical pregnancy rate (gestational sac and fetal heartbeat) and the live birth rate were used as the endpoints.</p></sec><sec><title>Results</title><p>Results. There were no differences in the implantation rate between groups 1, 2 and 3 (48.39%, 35.71% and 32%, respectively; p &gt; 0.05). The clinical pregnancy rate was significantly higher in group 1 than in group 2 (45.16% and 21.43%; p &lt; 0.05), in group 3 that was 32%. The difference in the live birth rate was not significant (33.3%, 19.05% and 24%; p &gt; 0.05). Among women who received IVIG within  the IVF (IVF/ICSI)  protocol and during the subsequent pregnancy (prior to 12 weeks), the the live birth rate was 83.3% (15/18), which was 33,3% higher (p &gt; 0.05) than for women who received IVIG only  within  the IVF (IVF/ICSI)  – 50% (6/12).</p></sec><sec><title>Conclusion</title><p>Conclusion. The use of IVIG in APA-positive women undergoing IVF (IVF/ICSI)  significantly increases the clinical pregnancy rate, but does not increase the implantation rate and the live birth rate. Further studies are needed to evaluate the efficacy of IVIG within  the IVF (IVF/ICSI) protocol  and the continued therapy during pregnancy.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Введение</title><p>Введение. Женщины с бесплодием и циркуляцией антифосфолипидных антител (АФА)  относятся к группе риска развития репродуктивных  потерь. Возможным  методом улучшения результативности протоколов  экстракорпорального оплодотворения   (ЭКО) у этих пациенток является иммуномодулирующая   терапия с применением внутривенных  иммуноглобулинов (ВВИГ).  Цель исследования: изучение эффективности  применения ВВИГ у женщин с наличием АФА при проведении протокола  ЭКО (ЭКО/ICSI).</p></sec><sec><title>Материалы  и методы</title><p>Материалы  и методы. Была обследована191 женщина: 128 женщин с циркуляцией АФА – 77 женщин получили ВВИГ в протоколе  ЭКО в курсовой дозе15 г (1 группа), 51 женщина не получала ВВИГ в протоколе   ЭКО (2 группа). а также  63 женщины без АФА (3 группа), планирующих  протокол ЭКО. Женщины 1 и 2 группы получали профилактические дозы низкомолекулярных гепаринов и низкие дозы ацетилсалициловой  кислоты. Из исследования исключены  женщины с низким качеством эмбрионов. В исследуемых группах оценивались: частота имплантации (положительный тест на ХГЧ), частота клинической беременности (выявление плодного яйца и сердцебиения плода) и частота рождения живых детей.</p></sec><sec><title>Результаты</title><p>Результаты. Между группами не было выявлено различий в частоте имплантации (48,39%, 35,71% и 32%, соотвественно; p&gt; 0,05). Частота клинической беременности была значимо выше в 1 группе по сравнению со2 (45,16% и 21,43%; p&lt; 0,05), а в 3 группе составила 32%. Частота регистрации клинической беременности от числа женщин с положительным  результатом анализа на β-ХГЧ составила  93,33%, 60% и 93,75% (p1,2&lt; 0,05, p2,3&lt; 0,05, p1,3&gt; 0,05, соответственно). Частота родов была сопоставима в трех группах  (33,3%, 19,05% и 24%; p&gt; 0,05). Среди женщин, получивших ВВИГ в протоколе  ЭКО и I триместре  беременности, частота родов составила 83,3% (15/18), что было на 33,3% больше (p&gt; 0,05)  по сравнению с женщинами, получившими ВВИГ только в протоколе   ЭКО – 50% (6/12).</p></sec><sec><title>Заключение</title><p>Заключение. Применение иммунотерапии   ВВИГ в протоколе   ЭКО (ЭКО/ICSI) у женщин с бесплодием и циркуляцией АФА значимо увеличивает частоту регистрации клинической  беременности, снижая  процент  доклинических потерь беременности,  но не увеличивает частоту  имплантации и частоту родов.
