All patients underwent examination and treatment in the Herzen Moscow Research Cancer Institute. Average age of patients was 35 + 5.6 years (21-67 years). Patients were divided int two groups: I-group included 114 woman, in which patients underwent surgical treatment and 56 patients on chemoradiation therapy, that preceded the surgery; II-group included 83 patients, in which the first stage of treatment was neoadjuvant therapy. MRI was performed in all patients within one week before radical surgery. MRI results have been compared with the histopathological report of the hysterectomy specimen. Results: The accuracy, sensitivity and specificity of MRI for perioperative evaluationof cervical cancer local staging in I group were 87%, 67%, 81%, positive predictive values (PPVs) and negative predictive values (NPVs) were 94% and 56%, respectively. For II group accuracy, sensitivity and specificity of MRI were 72%, 90%, 83%, PPVs and NPVs were 83%. Conclusion: MRI has a high diagnostic performance in preoperative staging of cervical cancer, content regardless of neoadjuvantt herapy. MRI allows to optimize the individual treatment plan in patients with cervical cancer.</p></abstract><trans-abstract xml:lang="ru"><p>Целью исследования являлось изучение возможностей магнитно-резонансной томографии (МРТ) в предоперационной оценке местной распространенности рака шейки матки. Материалы и методы: в исследование включены 253 больные раком шейки матки (РШМ), проходившие обследование и лечение в отделении онкогинегологии в период с ноября 2006 г. по февраль 2012 г., средний возраст которых составил 35+5,6 лет, при разбросе от 21 до 67 лет. В соответствии с методом лечения и последовательностью проведения хирургического этапа больные были подразделены на две группы: I группу составили 114 пациенток, которым было выполнено хирургическое лечение и 56 пациенток, у которых операция была первым этапом комбинированного или комплексного лечения; II группу составили 83 больные, на первом этапе лечения которым проводилась неоадьювантная химиолучевая терапия. Всем пациентам за неделю до хирургического лечения была выполнена МРТ. Результаты МРТ были сопоставлены с данными послеоперационного гистологического исследования. Результаты: диагностическая ценность МРТ в предоперационной оценке местной распространенности шейки матки в I группе составила: чувствительность, специфичность и точность - 87,67 и 81% соответственно. При этом прогностическая ценность положительного результата (ПЦП) соответствовала 86%, а прогностическая ценность отрицательного результата (ПЦО) - 68%. Во II группе чувствительность, специфичность и точность МРТ для оценки местной распространенности РШМ составили: 72,90 и 83%, ПЦП и ПЦО соответствовали 83%. Выводы: МРТ обладает высокой информативностью в предоперационном стадировании РШМ. Диагностическая ценность МРТ у больных РШМ, проходивших неоадьювантную химиолучевую терапию и у пациенток, которым на первом этапе проводилось оперативное лечение, является сопоставимой и способствует оптимизации оценки местной распространенности опухоли, что позволяет оптимизировать индивидуальный план лечебной тактики.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>рак шейки матки</kwd><kwd>МРТ</kwd><kwd>стадирование</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Cervical cancer</kwd><kwd>MRI</kwd><kwd>Staging</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Кудреватых Е.В.
