Such complications may profoundly worsen prognosis, impair quality of life, and hinder further treatment. Imaging diagnostics plays a crucial role in their timely detection, providing an opportunity not only to evaluate an extent and distribution of pathology but also to differentiate expected post-therapeutic changes from true complications. This article reviews the most common complications encountered after surgical procedures, such as lymphatic complications, fistulas, and infectious processes, as well as those following pelvic exenteration. Radiation-induced toxic effects are described, including both early and late changes affecting the gastrointestinal tract, urinary system, and musculoskeletal structures. Particular attention is paid to complications associated with chemotherapy and contemporary systemic therapies, including targeted agents and immunotherapy, which may result in hepatobiliary, pancreatic, gastrointestinal, vascular, and skeletal toxicities. Typical radiological manifestations related to such complications are discussed primarily outlining those following computed tomography (CT) and magnetic resonance imaging (MRI) exerting an essential role for early diagnosis, treatment planning, and follow-up. A comprehensive understanding of imaging features for abdominopelvic complications in patients with gynecologic malignancies is required for accurate diagnosis, prevented misinterpretation, and patient care optimization.</p></abstract><trans-abstract xml:lang="ru"><p>Злокачественные новообразования (ЗНО) женской репродуктивной системы сопровождаются высоким риском развития абдоминально-тазовых осложнений, связанных как с самим опухолевым процессом, так и с проводимой терапией, включая хирургическое вмешательство, лучевую терапию, химио- и иммунотерапию. Эти осложнения могут существенно ухудшать прогноз, снижать качество жизни пациенток и затруднять проведение последующего лечения. Ключевую роль в их своевременном выявлении играет лучевая диагностика, позволяющая не только оценить степень и распространенность патологического процесса, но и дифференцировать ожидаемые посттерапевтические изменения от истинных осложнений. В статье представлены основные виды осложнений, возникающих после хирургических вмешательств (лимфатические осложнения, свищи, инфекционные процессы), экзентерации органов малого таза, а также токсические эффекты лучевой терапии, включая ранние и поздние изменения со стороны желудочно-кишечного тракта, мочевыводящей системы и опорно-двигательного аппарата. Особое внимание уделено осложнениям, связанным с химиотерапией и современными методами системного лечения, включая таргетные препараты и иммунотерапию, которые могут приводить к повреждениям со стороны печени, поджелудочной железы, кишечника, сосудистой и костной систем. Рассмотрены типичные рентгенологические проявления осложнений при использовании компьютерной томографии и магнитно-резонансной томографии, что имеет решающее значение для ранней диагностики, выбора тактики лечения и последующего наблюдения.
