A prospective single-center cohort study was conducted. The control group comprised 40 women with uncomplicated pregnancies. The observation group consisted of 97 patients with a history of ePE, stratified based on pregnancy outcome into the comparison group (n = 59) characterized by a favorable gestational course, and the main group (n = 38) with relapsed еPE (30 patients were excluded from the study). Markers of systemic endotheliosis were determined in women at the PC stage and at 11–13 weeks of gestation: endothelin-1 (ET-1) and endothelial extracellular vesicles (EVs; CD-144).</p></sec><sec><title>Results</title><p>Results. At the PC stage, women in the control group and patients from the comparison group had comparable median (Me) ET-1 levels – 0.39 and 0.40 pmol/ml (p = 0.5935), respectively; at the same time, patients with relapsed еPE vs. control group had it significantly elevated (Me = 0.55 pmol/ml; p = 0.0382). At gestational age of 11–13 weeks, ET-1 level was significantly higher in the group with relapsed еPE (Me = 0.93 pmol/ml) than in control group (Me = 0.29 pmol/ml; p &lt; 0.0001) and comparison group (Me = 0.42 pmol/ml; p = 0.0003). No significant changes in E-EVs (CD-144) level at various PС stages were observed, whereas in the study groups such parameters remained differed.</p></sec><sec><title>Conclusion</title><p>Conclusion. Biological markers evidencing about systemic endothelial dysfunction/destruction – ET-1 and E-EVs (CD-144) in patients with previous ePE can be considered as predictive tests of disease relapse with an accuracy of 63.5 % and 83.0 % at the PС stage and 85.7 % and 94.2 % at gestational age of 11–13 weeks, respectively. </p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Цель</title><p>Цель: исследовать содержание биологических маркеров повреждения эндотелия в сыворотке крови у женщин с ранней преэклампсией (рПЭ) в анамнезе на прегравидарном (ПГ) этапе и в I триместре беременности.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено проспективное одноцентровое когортное исследование: группа контроля – 40 женщин с физиологическим течением беременности; группа наблюдения – 97 пациенток с рПЭ в анамнезе, распределенные по исходу наблюдаемой беременности в группу сравнения (n = 59) при благоприятном течении гестации и основную группу (n = 38) при рецидиве рПЭ (30 пациенток были исключены из исследования). У женщин на ПГ этапе и в 11–13 нед беременности определяли маркеры системного эндотелиоза: эндотелин-1 (ЭТ-1) и экстраклеточные микровезикулы эндотелиального происхождения (англ. endothelial extracellular vesicles, Е-EVs; СD-144).</p></sec><sec><title>Результаты</title><p>Результаты. На ПГ этапе у женщин группы контроля и пациенток группы сравнения определен сопоставимый уровень ЭТ-1 по медиане (Ме) – 0,39 и 0,40 пкмоль/мл (р = 0,5935), при этом в группе пациенток с рецидивом рПЭ показатель был значимо больше (Ме = 0,55 пкмоль/мл; р = 0,0382) относительно группы контроля. Уровень Е-EVs (СD-144) в основной группе (Me = 2,52 cоб/мкл) определен как значимо больший относительно и группы В сроке гестации 11–13 нед уровень ЭТ-1 был статистически значимо выше в группе с рецидивом pПЭ (Ме = 0,93 пкмоль/мл) относительно группы контроля (Ме = 0,29 пкмоль/мл; p &lt; 0,0001) и группы сравнения (Ме = 0,42 пкмоль/мл; p = 0,0003). Не определено статистически значимых изменений по количеству Е-EVs (СD-144) относительно ПГ этапа при сохраняющейся разнице показателей в обследованных группах. </p></sec><sec><title>Заключение</title><p>Заключение.
