Introduction. Aortic dissection (AD) is a rare and extremely dangerous pathology in obstetrics and gynecology. AD frequency in pregnant women comprises only 14.5 cases per 1 million subjects and leads to maternal death in 30 % cases. AD is characterized by the difficulty in performing differential diagnostics.

Aim

Aim: to describe a clinical case demonstrating the difficulties in timely AD diagnostics and treatment during delivery.

Materials and Methods

Materials and Methods. A rare clinical case of asymptomatic AD resulting in death of the woman in labor is presented after assessing the patient medical history, the conclusion of the pathoanatomical examination and the protocol of the mortality review.

Results

Results. The presented AD clinical case in labor with subtle clinical symptoms demonstrate the extreme difficulty of timely diagnostics of this pathology for practitioners, which inevitably resulted in maternal mortality.

Conclusion

Conclusion. AD during pregnancy and labor is the condition with extremely rare and difficult diagnostics, high probability of maternal and perinatal mortality, which should be known and remembered by obstetrician-gynecologists.

Введение. Расслоение аорты (РА) – редкая и чрезвычайно опасная патология в акушерско-гинекологической практике. Частота РА у беременных составляет всего 14,5 случаев на 1 млн и приводит к смерти матери в 30 % случаев. РА характеризуется сложностью дифференциальной диагностики.

Цель

Цель: на клиническом примере продемонстрировать трудности своевременной диагностики и лечения РА в родах.

Материалы и методы

Материалы и методы. Приведен редкий клинический случай бессимптомного расслоения аорты, повлекшего за собой смерть роженицы. Описание выполнено на основании истории болезни, заключения патологоанатомической экспертизы и протокола летальной комиссии.

Результаты

Результаты. Представленный клинический случай РА в родах со стертой клинической симптоматикой демонстрирует крайнюю сложность своевременной диагностики данной патологии у практических врачей, что закономерно привело к материнской летальности.

Заключение

Заключение. РА при беременности и в родах – крайне редкое и трудно диагностируемое состояние с высокой вероятностью материнской и перинатальной смертности, о которой необходимо знать и помнить врачу акушеру-гинекологу.
