A prospective controlled cohort non-randomized interventional study was conducted: within the years 2014 to 2020, there were examined 546 women with genital malignancies, divided into 2 groups: group I – 155 cancer patients with former thrombosis, group II – 391 women with female genital cancer without former thrombotic complications. Control group consisted of 137 patients with benign female genital tumors. The spectrum of circulating APA was studied: antibodies to â2-glycoprotein I (â2-GPI), annexin V and prothrombin as well as genetic thrombophilia due to mutations genes encoding factor V Leiden, methylenetetrahydrofolate reductase (MTHFR) including polymorphism in genes for prothrombin, platelet glycoproteins and plasminogen activator inhibitor-1 (PAI-1).</p></sec><sec><title>Results</title><p>Results. It was found that frequency of circulating APA as well as incidence rate of genetic thrombophilia between cancer patients from group I vs. group II significantly differed: APA in group I vs. group II was 86 (55.5 %; p &lt; 0.01) vs. 92 (23.5 %) compared to 7 (5.1 %) in control group. Genetic thrombophilia was dominated in group I by mutated MTHFR (92.9 %), polymorphismin PAI-1 (28.4 %) and platelet glycoprotein (44.5 %) that were significantly higher (p &lt; 0.05) compared to group II and control group. Hence, it allows to suggest that such identified thrombophilia markers are largely associated with a risk of developing thrombotic complications.</p></sec><sec><title>Conclusion</title><p>Conclusion. Detected high percentage of patients with circulating APA and genetic thrombophilia among cancer patients with former thromboembolic complications corroborate a role for genetic and acquired thrombophilia in developing pre-thrombotic condition. Detecting a range of circulating APA and genetic thrombophilia allows to identify patients who might be referred to a high risk of thrombogenesis and require to preventive application of anticoagulant therapy.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Введение</title><p>Введение. В рамках исследования, посвященного разработке стратегии профилактики тромботических осложнений у онкологических больных, была выдвинута гипотеза, что не только генетическая тромбофилия, но и приобретенные формы, в частности циркуляция антифосфолипидных антител (АФА), являются серьезным фактором риска тромбозов у онкологических больных.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: выявить частоту и спектр врожденных и приобретенных форм тромбофилии у онкогинекологических пациентов с раком яичников, тела матки и шейки матки.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено интервенционное проспективное контролируемое когортное нерандомизированное исследование: за период с 2014 по 2020 гг. были обследованы 546 женщин со злокачественными новообразованиями женских половых органов: группа I – 155 онкологических пациенток, у которых в анамнезе уже были эпизоды тромбозов; группа II – 391 онкологическая больная без тромботических осложнений в анамнезе. В качестве группы контроля обследованы 137 пациенток с доброкачественными опухолями женских половых органов. Был изучен спектр циркулирующих АФА – антитела к â2-гликопротеину I (â2-GPI), аннексину V и протромбину и генетические формы тромбофилии – мутации фактора V Leiden, гена метилентетрагидрофолатредуктазы (MTHFR), гена протромбина, полиморфизмы гена тромбоцитарных гликопротеинов и ингибитора активатора плазминогена-1 (PAI-1).</p></sec><sec><title>Результаты</title><p>Результаты. Выявлено достоверное различие в частоте циркуляции АФА и генетической формы тромбофилии между онкологическими больными групп I и II и контрольной группой: циркуляция АФА в группе I определена у 86 (55,5 %), что существенно выше (p &lt; 0,01), чем в группе II – 92 (23,5 %) и контрольной группе – 7 (5,1 %). Среди генетических форм тромбофилии в группе I наиболее часто регистрировали мутацию MTHFR (92,9 %), полиморфизм PAI-1 (28,4 %) и тромбоцитарного гликопротеина (44,5 %), что значимо выше (p &lt; 0,05), чем в группе II и контрольной группе, что позволяет говорить, о том, что выявленные маркеры тромбофилии в значительной степени ассоциированы с риском развития тромботических осложнений.</p></sec><sec><title>Заключение</title><p>Заключение. Выявление большого процента пациентов с циркуляцией АФА и генетических форм тромбофилии среди онкологических больных с тромбоэмболическими осложнениями в анамнезе подтверждает роль генетической и приобретенной тромбофилии в формировании предтромботического состояния.
